Indications & Clinical Uses

  • Timoptic 0.5% (timolol maleate ophthalmic solution) is a topical non-selective beta-blocker used to lower elevated pressure inside the eye in adults with ocular hypertension or open-angle glaucoma.
  • Treatment usually starts with the 0.25% strength twice daily; if the response is inadequate, the dose may be increased to 0.5% twice daily, and once pressure is controlled some patients are stepped down to once-daily dosing under medical supervision.
  • Timoptic is the original plain aqueous timolol solution and is a genuinely different formulation from the once-daily gel-forming products in the same family - Timoptic-XE and the generic Timolol GFS - so the two should never be swapped dose-for-dose without a doctor's guidance.
  • Timoptic is taken at home by the patient, not administered in a clinic.

Side Effects

  • Burning or stinging on instillation is the most frequently reported effect, affecting roughly one in eight patients; headache, mild bradycardia, nausea, dizziness and eye irritation are also reported.
  • Timoptic carries no boxed warning, but as a beta-blocker it can be absorbed into the bloodstream and, in susceptible people, cause slow or irregular heartbeat, worsening breathing difficulty or wheezing in asthma or COPD, and worsening of existing heart failure.
  • Seek urgent medical help for chest pain, severe shortness of breath, fainting, or a very slow pulse.
  • People with diabetes should be aware it can mask the warning signs of low blood sugar, and it can also mask a fast heartbeat caused by an overactive thyroid.

Precautions & Warnings

  • Timoptic must not be used by anyone with bronchial asthma or a history of it, severe chronic obstructive pulmonary disease, sinus bradycardia, second- or third-degree heart block, overt cardiac failure, cardiogenic shock, or hypersensitivity to any ingredient.
  • Use cautiously in diabetes, since it can hide hypoglycaemia symptoms, and in hyperthyroidism, since it can mask a rapid heartbeat.
  • Remove contact lenses before a dose and wait at least 15 minutes before putting them back in.
  • Pressing gently on the inner corner of the eye for about a minute after each drop reduces the amount of drug absorbed into the body.
  • Do not confuse the dosing schedule with the once-daily gel products that share the timolol name.

Avoid Interactions

  • Tell your healthcare provider and pharmacist about all medicines and supplements you use.
  • Taking Timoptic with an oral beta-blocker can add to its effects on heart rate and blood pressure, and combining it with certain calcium channel blockers raises the risk of heart conduction problems.
  • Catecholamine-depleting drugs such as reserpine can cause an additive fall in blood pressure and heart rate, and CYP2D6 inhibitors can increase systemic absorption of timolol.
  • Do not use Timoptic together with a second topical beta-blocker eye drop, since combining two products from the same class adds no extra benefit and increases the risk of side effects.
