Indications & Clinical Uses

  • Trimix is the common name for a compounded intracavernosal injection combining three vasoactive agents - alprostadil, papaverine and phentolamine - prepared to an individual prescription by a compounding pharmacy for men with erectile dysfunction (ED) who have not responded to, or cannot take, oral ED medicines.
  • There is no FDA-approved or DailyMed-listed label for this fixed triple combination; it is not a manufactured brand-name product but a physician-prescribed compounded mixture, so strengths and ratios of the three ingredients vary by prescription and by pharmacy.
  • This content is based on the FDA label for the single-agent product Caverject (alprostadil), which shares Trimix's main active ingredient and its intracavernosal route, together with general urology guidance on penile injection therapy.
  • Trimix is injected directly into the side of the penis 5 to 20 minutes before intended sexual activity, after an initial in-office dose is set by a healthcare provider.

Side Effects

  • The most common reaction, reported in roughly a third of alprostadil injection users, is penile pain at the injection site, usually mild and temporary.
  • The most serious risk shared by all Trimix components is priapism - an erection lasting more than 4 hours - which can permanently damage penile tissue if not treated urgently; seek emergency care immediately if an erection does not resolve.
  • Repeated injections can cause bruising, small nodules or scarring (fibrosis) at the injection site, sometimes leading to curvature of the penis (Peyronie's-like changes); tell your provider if you feel lumps or notice bending.
  • Papaverine, one of Trimix's three ingredients, is not itself approved for penile injection and its manufacturer's label specifically warns that intracorporeal use has caused persistent priapism needing surgery.
  • There is no boxed warning on the individual component labels reviewed, but the priapism risk is prominently and repeatedly flagged.

Precautions & Warnings

  • Trimix must be started and dose-titrated in a healthcare provider's office, where you will stay until any test erection subsides, and never used more than once in 24 hours or more than about 3 times a week.
  • It must not be used by men with conditions that predispose to prolonged erections, such as sickle cell disease, leukaemia or multiple myeloma, or by men with penile implants, Peyronie's disease or significant penile deformity/fibrosis.
  • Because papaverine (one of the three actives) is specifically not approved for intracavernosal injection and phentolamine can cause blood pressure swings, Trimix should only be used exactly as compounded and dosed by your prescriber - never adjust the mixture or volume yourself.
  • People taking blood thinners (such as warfarin or heparin) have a higher risk of bleeding or bruising at the injection site and should apply firm pressure afterwards.
  • This is a human urology treatment and is not related to any veterinary anaesthetic mixture that shares the name 'trimix'.

Avoid Interactions

  • Anticoagulant and antiplatelet medicines, including warfarin, heparin and aspirin, increase the risk of bleeding, bruising or prolonged oozing at the Trimix injection site.
  • Other medicines that lower blood pressure can add to phentolamine's blood-pressure-lowering effect and increase dizziness or fainting, especially on standing up.
  • Combining Trimix with oral erectile dysfunction medicines (such as PDE5 inhibitors) or other injectable or intraurethral ED treatments is not recommended without your prescriber's guidance, since this can raise the risk of a prolonged erection.
  • Papaverine, taken with other drugs that affect liver enzymes, has rarely been linked to liver injury with repeated use, so report unusual fatigue, dark urine or yellowing skin.
  • Because Trimix is individually compounded, always tell the prescribing clinician and compounding pharmacist about every medicine, herbal product and supplement you take.
