Indications & Clinical Uses
- Veltassa (patiromer, as patiromer sorbitex calcium) is a potassium binder supplied as a powder that you mix into a suspension to drink.
- The 8.4 g packet is the standard adult starting dose.
- It is indicated for the treatment of hyperkalaemia (high blood potassium) in adults and in children aged 12 years and older.
- Veltassa should not be used as an emergency treatment for life-threatening hyperkalaemia.
- Adults start at 8.4 g once daily and the dose is adjusted by 8.4 g at weekly intervals to a maximum of 25.2 g daily.
- Children aged 12 to 17 start at 4 g once daily, adjusted by 4 g weekly.
- Other oral medicines must be taken at least 3 hours before or after Veltassa.
Side Effects
- The most common side effects are constipation (7.2%), low blood magnesium (hypomagnesaemia, 5.3%), diarrhoea (4.8%), nausea (2.3%), abdominal discomfort (2.0%) and flatulence (2.0%).
- Tell your healthcare provider if constipation is severe or does not go away, or if you have severe abdominal pain, swelling or vomiting, as Veltassa should be avoided in severe constipation, bowel obstruction or impaction.
- Nausea, abdominal discomfort and flatulence each occur in about 2% of patients.
- The Veltassa label carries no boxed warning.
- The effect on blood potassium is checked by blood tests, and the dose is adjusted at weekly intervals.
Precautions & Warnings
- Serum magnesium should be monitored, and magnesium supplements considered if levels fall.
- Do not use Veltassa in severe constipation, bowel obstruction or impaction, or if you are allergic to patiromer or its components.
- It is not an emergency treatment for high potassium.
- Veltassa is not absorbed into the body, so harm to an unborn baby is not expected.
- Prepare each dose just before taking it: stir the powder into about one-third of a cup of water or soft food, expect a cloudy mixture that does not dissolve, and drink it at once.
- Do not heat it or take the powder dry.
- Store in a refrigerator at 2 to 8 degrees C; once at room temperature use within 3 months.
Avoid Interactions
- Veltassa can bind other oral medicines in the gut and reduce their absorption, so take other oral medicines at least 3 hours before or 3 hours after Veltassa.
- The label lists medicines with reduced absorption or exposure including ciprofloxacin, levothyroxine, metformin, mycophenolate mofetil, quinidine, thiamine, telmisartan and the beta-blockers bisoprolol, carvedilol and nebivolol.
- Medicines that increase Veltassa toxicity were not identified in the label.
- Because separation of doses is the key safeguard, ask your pharmacist if you are unsure when to take another medicine.
- Tell your healthcare provider about all prescription medicines, over-the-counter products and supplements you take.

Frequently Asked Questions
Veltassa (patiromer) treats hyperkalaemia, or high blood potassium, in adults and children aged 12 and over. It is a powder mixed into a drink, and it binds potassium in the gut so it leaves in stool. Veltassa is not an emergency treatment for life-threatening high potassium.
Mix Veltassa (patiromer) powder with water or soft food just before taking, and drink it straight away. Adults start with 8.4 g once daily; children aged 12 to 17 start with 4 g daily. Never heat it or take it dry, and take other oral medicines 3 hours apart.
Veltassa (patiromer) side effects include constipation (7.2%), low magnesium (5.3%), diarrhoea (4.8%), nausea, stomach discomfort and gas. Report severe constipation, severe stomach pain, swelling or vomiting to your prescriber. Veltassa (patiromer) has no boxed warning, and the effect on potassium is checked with blood tests.
Avoid taking other oral medicines within 3 hours of Veltassa (patiromer) because it can bind them, including ciprofloxacin, levothyroxine, metformin and some beta-blockers. Avoid Veltassa in severe constipation or bowel obstruction. Never heat the mixture or take the powder dry.
Store Veltassa (patiromer) in the refrigerator at 2°C to 8°C. If kept at room temperature it must be used within 3 months. Do not use it after the expiry date. Prepare each dose just before taking it, not in advance. Check the expiry date on each packet.
Veltassa (patiromer) can lower blood magnesium, seen in about 5.3% of patients, so magnesium is monitored and supplements may be added if it falls. Potassium is also checked regularly, and the dose is adjusted at weekly intervals. Veltassa is not absorbed into the body.
