Indications & Clinical Uses
- Wegovy 2.4 mg (semaglutide) is a glucagon-like peptide-1 (GLP-1) receptor agonist given as a once-weekly injection under the skin.
- In the US it is approved to reduce the risk of major adverse cardiovascular events in adults with established cardiovascular disease and obesity or overweight, to reduce excess body weight in adults and children aged 12 years and older with obesity (and in adults with overweight plus at least one weight-related condition), and to treat noncirrhotic MASH with moderate to advanced liver fibrosis (F2-F3) in adults.
- It is a weight-management medicine, not a diabetes treatment; Ozempic also contains semaglutide but is for type 2 diabetes.
- Strength warning: Wegovy pens come as 0.25, 0.5, 1, 1.7 and 2.4 mg, plus a 7.2 mg Wegovy HD pen, and Wegovy tablets (1.5, 4, 9 and 25 mg) also exist; follow the prescribed pen and dose.
Side Effects
- BOXED WARNING: in rodents, semaglutide causes thyroid C-cell tumours at clinically relevant exposures, and it is unknown whether Wegovy causes thyroid C-cell tumours, including medullary thyroid carcinoma, in humans.
- The most common side effects (reported by at least 5% of users) are nausea (44%), diarrhoea (30%), vomiting (24%), constipation (24%), abdominal pain (20%), headache (14%), fatigue (11%) and indigestion (9%).
- Serious reactions include acute pancreatitis and gallbladder disease, low blood sugar (especially with insulin or sulfonylureas), acute kidney injury, allergic reactions, diabetic retinopathy complications, a faster heart rate, and suicidal thoughts or behaviour.
- Seek urgent help for severe stomach pain or signs of an allergic reaction, and rotate injection sites between abdomen, thigh and upper arm.
Precautions & Warnings
- Wegovy carries a boxed warning for thyroid C-cell tumours and must not be used if you or a family member has had medullary thyroid carcinoma, or if you have Multiple Endocrine Neoplasia syndrome type 2 (MEN 2), or have had a serious allergic reaction to semaglutide.
- Treatment starts at 0.25 mg weekly for 4 weeks, then increases every 4 weeks (0.5 mg, 1 mg, 1.7 mg) to a maintenance dose of 2.4 mg or 1.7 mg weekly.
- It may cause fetal harm: stop when pregnancy is recognised, and stop at least 2 months before a planned pregnancy because it stays in the body for a long time.
- Watch blood sugar if you have type 2 diabetes, and tell your provider about mood changes.
- Store unused pens in the refrigerator at 2°C to 8°C; they may be kept at up to 30°C for up to 30 days.
Avoid Interactions
- Increase effect or toxicity: insulin and insulin secretagogues such as sulfonylureas increase the risk of low blood sugar, so doses may need adjusting.
- Wegovy delays stomach emptying, which may change how well other medicines taken by mouth are absorbed; ask your healthcare provider or pharmacist about timing of oral medicines.
- Reduce efficacy: no specific medicines are named in the label as reducing its effect.
- Tell your healthcare provider about all medicines, vitamins and supplements you take.
- Do not use Wegovy tablets, the 7.2 mg pen or the FlexTouch pen in place of your prescribed 2.4 mg pen unless your prescriber tells you to.

Frequently Asked Questions
Wegovy (semaglutide) is a weekly injection for weight management. It reduces excess body weight in adults and children aged 12 and over with obesity, and in adults with overweight plus a weight-related condition. It also lowers heart attack and stroke risk in adults with heart disease, and treats MASH liver fibrosis.
Inject Wegovy (semaglutide) once a week under the skin of the abdomen, thigh or upper arm, rotating sites. Start at 0.25 mg for 4 weeks, then step up every 4 weeks through 0.5, 1 and 1.7 mg to a maintenance dose of 2.4 mg or 1.7 mg.
The most common Wegovy side effects are nausea, diarrhoea, vomiting, constipation and abdominal pain. Serious ones include pancreatitis, gallbladder disease, kidney injury and low blood sugar. Wegovy carries a boxed warning for thyroid C-cell tumours seen in rodents. Report severe stomach pain or signs of an allergic reaction straight away.
Do not use Wegovy (semaglutide) if you or your family has had medullary thyroid carcinoma or MEN 2, or if you are allergic to semaglutide. Insulin and sulfonylureas raise low blood sugar risk with Wegovy. Avoid pregnancy and stop 2 months before planning one.
Store unused Wegovy (semaglutide) pens in the refrigerator at 2°C to 8°C. According to the US label, they can be kept at room temperature up to 30°C for up to 30 days. Do not confuse the 2.4 mg pen with other strengths or Wegovy tablets.
The Wegovy (semaglutide) boxed warning says semaglutide causes thyroid C-cell tumours in rodents at clinically relevant exposures; it is unknown whether it does in humans. Wegovy must not be used with a personal or family history of medullary thyroid carcinoma or MEN 2.
