Indications & Clinical Uses
- Wellbutrin SR 150 mg sustained-release tablets contain bupropion hydrochloride, an antidepressant, and are approved in the US for major depressive disorder (MDD) only.
- The category on your sheet is wrong: Wellbutrin SR is not approved for smoking cessation, although it contains the same active ingredient as Zyban, the separate smoking-cessation product, and some generic bupropion SR tablets are labelled for smoking cessation instead.
- Do not swap Wellbutrin SR with immediate-release bupropion or with Wellbutrin XL, which are dosed differently.
- The tablet is swallowed whole.
- The usual start is 150 mg once daily in the morning, raised after 3 days to 300 mg a day as 150 mg twice daily, at least 8 hours apart; the maximum is 400 mg a day (200 mg twice daily).
Side Effects
- The most common side effects (5% or more of patients) were headache (26%), dry mouth (17%), nausea (13%), difficulty sleeping (11%), constipation (10%), dizziness (7%), sweating and ringing in the ears (6% each), tremor (6%), anxiety (5%), rash (5%) and loss of appetite (5%).
- BOXED WARNING: antidepressants increased the risk of suicidal thoughts and behaviour in children, adolescents and young adults in short-term trials.
- Serious effects include seizures (dose-related, about 0.1% at 300 mg a day and 0.4% at 400 mg a day), raised blood pressure, angle-closure glaucoma, mania or hypomania, and psychosis or other neuropsychiatric reactions.
- Seek urgent help for a seizure, thoughts of self-harm, severe agitation or eye pain.
Precautions & Warnings
- The boxed warning requires close monitoring of everyone starting an antidepressant for worsening mood, agitation or suicidal thoughts, especially in young people and after dose changes.
- Wellbutrin SR is contraindicated in seizure disorder, current or past bulimia or anorexia nervosa, abrupt stopping of alcohol, benzodiazepines, barbiturates or antiepileptic drugs, use of an MAO inhibitor within 14 days (or linezolid or intravenous methylene blue), and bupropion hypersensitivity.
- Blood pressure should be checked, and bipolar disorder screened for before starting.
- In moderate to severe liver impairment the dose is 100 mg a day or 150 mg every other day; consider dose reduction in kidney impairment.
- A pregnancy registry exists (1-844-405-6185).
- Bupropion passes into breast milk, and seizures in breastfed infants have been reported.
Avoid Interactions
- Medicines that increase risk or bupropion effects: MAO inhibitors (contraindicated; hypertensive reaction), levodopa and amantadine (CNS toxicity), drugs that lower the seizure threshold, and alcohol, which should be minimised or avoided.
- Bupropion blocks CYP2D6, so levels of venlafaxine, antipsychotics, beta-blockers and Type 1C antiarrhythmics can rise and doses may need reducing.
- Medicines that reduce bupropion effect: CYP2B6 inducers such as ritonavir, efavirenz, carbamazepine, phenobarbital and phenytoin may lower levels; bupropion may lower digoxin levels and may reduce the effect of tamoxifen.
- Tell your healthcare provider about all medicines and supplements you use.

Frequently Asked Questions
Wellbutrin SR (bupropion) is an antidepressant approved in the US for major depressive disorder. It is not approved for quitting smoking, even though Zyban contains the same active ingredient. Wellbutrin SR 150 mg is a sustained-release tablet, taken once or twice daily.
Swallow Wellbutrin SR (bupropion) tablets whole; do not crush, divide or chew them. The usual start is 150 mg once daily in the morning, increased after 3 days to 150 mg twice daily. Doses must be at least 8 hours apart. The maximum is 400 mg a day.
Wellbutrin SR (bupropion) commonly causes headache, dry mouth, nausea, difficulty sleeping, constipation and dizziness. Seek urgent help for a seizure, thoughts of suicide, severe agitation, hallucinations or eye pain. It carries a boxed warning for suicidal thoughts and behaviour in children, adolescents and young adults.
Avoid taking Wellbutrin SR (bupropion) with MAO inhibitors, within 14 days of stopping one, or if you have a seizure disorder or an eating disorder such as bulimia or anorexia. Minimise or avoid alcohol, and do not stop alcohol, benzodiazepines or antiepileptics suddenly while taking it.
No. Wellbutrin SR (bupropion) is a sustained-release tablet taken in one or two doses a day, while Wellbutrin XL is extended-release and taken once daily in the morning. Immediate-release bupropion is also different. Do not switch between them without your healthcare provider setting the dose.
Wellbutrin SR (bupropion) data in pregnancy did not show an overall increase in birth defects, but some studies raised questions about heart defects, and untreated depression also carries risks. Tell your healthcare provider if you are pregnant or plan to be. A pregnancy registry exists (1-844-405-6185).
