Abiraterone (Abiraterone Acetate) is an oral hormonal therapy widely used in the treatment of advanced prostate cancer, particularly metastatic castration resistant prostate cancer (mCRPC) and metastatic hormone sensitive prostate cancer (mHSPC). It works by targeting androgen production, which plays a crucial role in the growth and progression of prostate cancer cells.
Abiraterone acetate is a prodrug that is converted in the body to this, an inhibitor of the CYP17 enzyme. This enzyme is essential for androgen synthesis in the testes, adrenal glands, and prostate tumor tissue. By blocking CYP17, this significantly reduces testosterone and other androgen levels, helping to slow or stop cancer progression. Due to its mechanism of action, abiraterone is typically prescribed in combination with low dose corticosteroids (such as prednisone) to prevent side effects related to mineralocorticoid excess.
Clinically, it has been shown to improve overall survival, progression free survival, and quality of life in patients with advanced prostate cancer. It is commonly used either before or after chemotherapy, depending on disease stage and patient condition. The medication is taken orally, usually once daily on an empty stomach, as food can significantly increase drug absorption and risk of adverse effects.
Common side effects of it includes hypertension, hypokalemia (low potassium levels), fluid retention, fatigue, joint pain, and liver function abnormalities. Regular monitoring of blood pressure, electrolytes, and liver enzymes is recommended during treatment. When used under proper medical supervision, abiraterone remains a cornerstone therapy in modern prostate cancer management, offering effective androgen suppression beyond traditional hormone therapies.
