Diaprel is an oral antidiabetic medication whose active ingredient is gliclazide, a sulfonylurea class drug used to manage type 2 diabetes mellitus when diet, exercise, and weight loss alone are insufficient to control blood glucose levels. It works by stimulating the pancreatic β cells to release more insulin, which helps lower elevated blood sugar in people whose pancreatic function still remains.
How it works: Gliclazide binds to sulfonylurea receptors on pancreatic β cells, closing ATP sensitive potassium channels and opening calcium channels. The resulting calcium influx triggers insulin release, reducing blood glucose. It also enhances peripheral insulin sensitivity and decreases hepatic glucose production.
Dosage: Diaprel is typically taken once daily with breakfast to minimize the risk of low blood sugar (hypoglycemia) and ensure the drug’s peak effect coincides with food intake. Modified release formulations (e.g., Diaprel MR 30 mg or 60 mg) are common, and doses are individualized based on blood glucose response, starting low and titrating gradually. Always follow your doctor’s instructions.
Side effects: The most common adverse effect is hypoglycemia symptoms include sweating, dizziness, tremors, hunger, and confusion. Gastrointestinal issues like nausea, indigestion, diarrhea or constipation may occur. Skin reactions and very rare blood disorders have been reported.
Precautions and contraindications: Diaprel should not be used in type 1 diabetes, diabetic ketoacidosis, severe kidney or liver disease, or during pregnancy or breastfeeding. Caution is needed when combined with other medications that affect blood sugar and in patients with irregular eating patterns or alcohol use.
