Indications & Clinical Uses

  • Doxycycline (Teva) 100 mg capsules contain doxycycline, a broad-spectrum tetracycline antibiotic.
  • It treats infections caused by susceptible organisms, including rickettsial infections such as Rocky Mountain spotted fever, Q fever and typhus; respiratory infections including Mycoplasma pneumoniae; chlamydia, urethritis, and gonorrhoea or syphilis when penicillin cannot be used; plague, tularaemia and cholera; acute intestinal amoebiasis as an adjunct; and severe acne as add-on therapy.
  • It is also approved to prevent malaria caused by Plasmodium falciparum in short-term travellers (under 4 months) to areas with chloroquine or pyrimethamine-sulfadoxine resistance.
  • The product sheet lists the category as antimalarial, but malaria prevention is only one of many approved uses and this is primarily an antibacterial.
  • Capsules generally contain doxycycline hyclate while some tablets contain doxycycline monohydrate; both deliver an equivalent 100 mg of doxycycline.
  • Teva supplies doxycycline in several markets.

Side Effects

  • Doxycycline (Teva) has NO boxed warning.
  • Nausea and vomiting are the commonest effects; diarrhoea, loss of appetite, indigestion and vaginal thrush occur less often.
  • Photosensitivity - an exaggerated sunburn reaction on sun-exposed skin - is well recognised, and the capsules should be stopped at the first sign of skin redness.
  • Rare but important reactions include oesophagitis and oesophageal ulceration, usually when a capsule is swallowed with too little water or just before lying down; Clostridioides difficile-associated diarrhoea, which can appear during or months after treatment; benign intracranial hypertension causing headache, blurred or double vision and, rarely, permanent visual loss; severe skin reactions including Stevens-Johnson syndrome and toxic epidermal necrolysis; hypersensitivity reactions up to anaphylaxis; and blood problems such as haemolytic anaemia and low platelets.
  • Get urgent help for watery diarrhoea with fever, a spreading blistering rash, severe headache with visual change, or pain on swallowing.

Precautions & Warnings

  • Doxycycline is contraindicated if you are allergic to tetracyclines, and the UK label also contraindicates it in pregnancy and in breastfeeding mothers.
  • It is not given to children under 8 years (under 12 years on the UK label) except for severe or life-threatening infections such as rickettsial disease, because tetracyclines can permanently stain developing teeth and may slow bone growth.
  • Swallow the capsule whole with a full glass of water while sitting or standing upright, and not immediately before going to bed, to prevent oesophageal ulceration; taking it with food reduces stomach upset without stopping it working.
  • Use sunscreen and cover up, because sunburn happens more easily.
  • People who are overweight or who have had intracranial hypertension are at higher risk of it recurring.
  • Complete the full course even once you feel better.
  • For malaria prevention, doxycycline does not replace repellent, nets and covering up.
  • Store below 25 degrees C in the original container; the US label allows 20 to 25 degrees C.

Avoid Interactions

  • Tell your provider and pharmacist about everything you take.
  • Absorption of doxycycline is reduced by antacids containing aluminium, calcium or magnesium, by iron and zinc salts, by bismuth subsalicylate and by milk and dairy taken at the same time - separate these by 2 to 3 hours.
  • Barbiturates, carbamazepine and phenytoin shorten doxycycline's half-life and lower its effect.
  • Doxycycline increases the effect of warfarin and other anticoagulants, so the dose may need reducing and clotting monitored.
  • It may reduce the reliability of combined oral contraceptives, so use an additional method during and just after the course.
  • Avoid combining it with penicillins, whose bactericidal action is antagonised.
  • Retinoids such as isotretinoin and acitretin must not be taken with doxycycline because of raised intracranial pressure.
  • Ciclosporin levels may rise.
  • Methoxyflurane anaesthesia with tetracyclines has caused fatal kidney toxicity - tell your anaesthetist.
