Indications & Clinical Uses
- Lopressor 100mg tablets contain metoprolol tartrate, a beta1-selective (cardioselective) beta-blocker.
- They are approved to lower blood pressure in adults with hypertension, which reduces the risk of stroke and heart attack; to treat angina pectoris by reducing the number of attacks and improving exercise tolerance; and, in haemodynamically stable patients, to reduce cardiovascular mortality after a definite or suspected myocardial infarction.
- Metoprolol tartrate is the immediate-release salt and is normally taken twice daily with or immediately after food.
- It is not the same product as metoprolol succinate extended-release (Toprol-XL), which is taken once daily; the two are not interchangeable milligram-for-milligram or in dosing frequency.
- The current US label holder is Validus Pharmaceuticals, not Novartis, although Novartis originated the brand.
- Lopressor is also supplied as an injection for hospital use.
Side Effects
- Common side effects of Lopressor (metoprolol tartrate) include tiredness, dizziness, a slow heart rate, low blood pressure, cold hands and feet, shortness of breath, headache, depression, confusion, nightmares, diarrhoea and itching or rash.
- This label carries no boxed warning, but it warns that abruptly stopping metoprolol can cause a severe flare of angina, a heart attack or dangerous heart rhythms, so the dose must be reduced gradually over one to two weeks under supervision.
- Get urgent help for fainting, a pulse persistently below about 50 beats per minute, new or worsening breathlessness, swelling of the ankles with sudden weight gain, or wheezing.
- In people with diabetes, metoprolol can mask the warning signs of low blood sugar (hypoglycaemia), such as a racing heartbeat, so check glucose more often.
Precautions & Warnings
- Lopressor must not be used in severe bradycardia, second- or third-degree heart block, sick sinus syndrome without a pacemaker, cardiogenic shock, decompensated heart failure or known hypersensitivity to metoprolol.
- Use with care in bronchospastic disease such as asthma, where the lowest possible dose should be used and a bronchodilator kept available; in peripheral vascular disease; in phaeochromocytoma, where an alpha-blocker must be given first; and in liver impairment, since metoprolol is cleared hepatically.
- In thyrotoxicosis, abrupt withdrawal can precipitate thyroid storm.
- Patients on beta-blockers may not respond normally to adrenaline given for a severe allergic reaction.
- Blood pressure, pulse and heart failure symptoms should be monitored, and surgical or anaesthetic teams told you take it.
- Dizziness and tiredness can affect driving.
- Store at 25°C, protected from moisture and heat.
Avoid Interactions
- The effect and toxicity of Lopressor are increased by CYP2D6 inhibitors such as quinidine, fluoxetine, paroxetine, bupropion, terbinafine and ritonavir, which raise metoprolol blood levels.
- Additive slowing of the heart and falls in blood pressure occur with digoxin, clonidine, diltiazem, verapamil, other beta-blockers, amiodarone and anaesthetic agents; if clonidine is being stopped, the beta-blocker must be withdrawn several days first to avoid rebound hypertension.
- Catecholamine-depleting drugs such as reserpine add to bradycardia and hypotension, and alcohol may increase dizziness.
- Blood-pressure efficacy may be reduced by non-steroidal anti-inflammatory drugs.
- Metoprolol can blunt the response to adrenaline used for anaphylaxis and can mask the fast pulse caused by hypoglycaemia in people using insulin or sulfonylureas.
- Tell your healthcare provider about all medicines, including eye drops and supplements.

Frequently Asked Questions
Lopressor 100 mg (metoprolol tartrate) is a beta-blocker used to lower high blood pressure, to prevent angina attacks and improve exercise tolerance, and to reduce the risk of death after a heart attack in stable patients. It slows the heart and reduces how hard it has to work.
Lopressor (metoprolol tartrate) tablets are usually taken twice daily, with or immediately after food, at the same times each day. Swallow the tablet whole. Do not stop taking it suddenly: the dose must be lowered gradually over one to two weeks with medical supervision.
Common Lopressor (metoprolol tartrate) side effects are tiredness, dizziness, a slow pulse, low blood pressure, cold hands and feet, breathlessness, headache, depression and vivid dreams. Get urgent help for fainting, a pulse under about 50 beats per minute, new wheezing, or swollen ankles with sudden weight gain.
Avoid stopping Lopressor suddenly, and avoid combining it with verapamil or diltiazem unless your healthcare provider agrees, because the heart can slow dangerously. Alcohol adds to dizziness. CYP2D6 inhibitors such as fluoxetine, paroxetine and bupropion raise metoprolol tartrate levels; anti-inflammatory painkillers can blunt its blood-pressure effect.
No. Lopressor is metoprolol tartrate, taken twice daily, while Toprol-XL is metoprolol succinate extended-release, taken once daily. They are not interchangeable milligram-for-milligram or in dosing frequency, and switching without a prescriber's instruction risks under-treatment or excessive beta-blockade. Check which salt your prescription names.
Stopping Lopressor (metoprolol tartrate) abruptly can cause a severe flare of angina, a heart attack or dangerous heart rhythms, especially in people with coronary artery disease. The dose should be reduced gradually over one to two weeks with monitoring. In thyroid overactivity, sudden withdrawal can trigger thyroid storm.
