Indications & Clinical Uses
- Mavik (trandolapril) 4 mg tablets are DISCONTINUED in the United States: AbbVie stopped marketing the Mavik brand for business reasons, not for safety or effectiveness, so trandolapril is now available in the US only as a generic.
- Mavik was an angiotensin-converting enzyme (ACE) inhibitor; when it was marketed it was used to treat high blood pressure, alone or combined with other blood-pressure medicines such as a thiazide diuretic, and to treat left-ventricular dysfunction or heart failure in adults who were clinically stable after a recent heart attack.
- It was approved for adults only; safety and effectiveness in children were not established.
- Trandolapril is also sold in some countries combined with verapamil under the brand name Tarka.
Side Effects
- Mavik carries a boxed warning for fetal toxicity: trandolapril can injure or kill a developing baby, so it must be stopped as soon as pregnancy is detected.
- The most common side effects are cough, dizziness, and diarrhea; in patients treated after a heart attack, cough, dizziness, and low blood pressure were more frequent.
- Other reported effects include headache, fatigue, and an altered or metallic taste.
- Rarely, ACE inhibitors including trandolapril can cause angioedema - sudden swelling of the face, lips, tongue, or throat that can block breathing - a medical emergency requiring immediate treatment and permanent discontinuation.
- Report any persistent cough, swelling, signs of high potassium such as muscle weakness or an irregular heartbeat, or yellowing of the skin or eyes to your healthcare provider promptly.
Precautions & Warnings
- Do not take Mavik/trandolapril if you have a history of angioedema linked to any ACE inhibitor, or hereditary or idiopathic angioedema.
- Because of the boxed warning for fetal toxicity, it must not be used during pregnancy; stop treatment and contact your healthcare provider immediately if you become pregnant, and effective contraception is advised for anyone who could become pregnant.
- Kidney function and blood potassium should be monitored before and during treatment, especially with kidney impairment, diabetes, or heart failure, or when also taking potassium-sparing diuretics or potassium supplements.
- Use with caution in liver disease and in people who are dehydrated or on a salt-restricted diet, since symptomatic low blood pressure can occur, particularly after the first dose.
- It can cause dizziness, so take care when driving or using machinery until you know how it affects you.
Avoid Interactions
- Avoid Interactions: combining trandolapril with potassium-sparing diuretics (such as spironolactone or amiloride), potassium supplements, or salt substitutes can cause dangerously high blood potassium.
- NSAIDs and COX-2 inhibitors, including regular ibuprofen use, can reduce its blood-pressure-lowering effect and increase the risk of kidney problems, especially in older adults or those with reduced kidney function.
- Combining an ACE inhibitor with an angiotensin receptor blocker or the direct renin inhibitor aliskiren (particularly in diabetes or kidney disease) increases the risk of low blood pressure, high potassium, and kidney injury, and is generally avoided.
- Lithium levels can rise to toxic levels when taken with trandolapril.
- Diabetes medicines, including insulin, may have an enhanced blood-sugar-lowering effect.
- Tell your healthcare provider about all medicines, including over-the-counter painkillers and supplements, before starting.

Frequently Asked Questions
Mavik (trandolapril) is an ACE inhibitor once used to treat high blood pressure and heart failure after a heart attack. The Mavik brand has been discontinued in the US for business reasons, though generic trandolapril is still available and works the same way.
Mavik tablets were taken by mouth once daily, with or without food, usually starting at a low dose that your healthcare provider increased gradually. It could be used alone or combined with other blood pressure medicines such as a diuretic.
The most common effects are cough, dizziness, and diarrhea. Mavik carries a boxed warning because ACE inhibitors like trandolapril can harm an unborn baby, and rarely cause angioedema - sudden swelling of the face or throat - which needs emergency care.
Avoid potassium supplements, salt substitutes, and potassium-sparing diuretics, which can raise blood potassium to unsafe levels. Regular use of NSAID painkillers can reduce the drug's blood-pressure effect. Do not combine it with aliskiren if you have diabetes or kidney disease.
Yes. Generic trandolapril contains the same active ingredient, strength, and dosing as the discontinued Mavik brand, and meets the same FDA quality and effectiveness standards. Ask your pharmacist if your prescription has switched from Mavik to a generic version.
No. Trandolapril can cause fetal injury or death, especially in the second and third trimesters, and must be stopped as soon as pregnancy is confirmed. Tell your healthcare provider right away if you are pregnant or planning pregnancy while taking it.
