Repaglinide is an oral antidiabetic agent that belongs to the class of meglitinides and is used to support blood sugar control in adults with type 2 diabetes when diet and exercise alone aren’t sufficient. It is known for its rapid onset and short duration of action, making it particularly effective at managing post‑meal glucose spikes.
The way repaglinide works involves stimulating the pancreas to release more insulin. It closes ATP‑dependent potassium channels on pancreatic beta cells, which leads to cell depolarization and the opening of calcium channels. The resulting calcium influx triggers the release of insulin into the bloodstream, helping the body lower blood glucose after meals. Because its action is quick and short‑lived, repaglinide is taken before eating and can be skipped if a meal is skipped.
Repaglinide is absorbed rapidly from the gastrointestinal tract, reaching peak levels in the blood within about an hour, and is primarily metabolised by liver enzymes. Its metabolites are excreted mainly via bile, with a small amount eliminated in urine.
Common effects associated with repaglinide include low blood sugar levels (hypoglycemia), nausea, headache, diarrhea, and joint discomfort. Because its action closely follows meal‑related glucose increases, the risk of prolonged low blood sugar between meals or overnight is lower than with some older glucose‑lowering agents.
