Indications & Clinical Uses
- Ponvory 2mg (the first tablet of a 14-day titration starter pack) contains ponesimod, a sphingosine 1-phosphate (S1P) receptor modulator taken once daily by mouth.
- It is approved in adults for relapsing forms of multiple sclerosis (MS), including clinically isolated syndrome, relapsing-remitting disease, and active secondary progressive disease; the EU approval is worded more narrowly, covering adults with relapsing MS that has active disease shown by recent relapses or new lesions on imaging.
- Treatment always starts with this 14-tablet titration pack, in which the dose rises step by step from 2mg up to 9mg, followed by a fixed 20mg maintenance tablet once daily from day 15 onward.
- Ponesimod reduces the number of lymphocytes able to leave the lymph nodes and reach the central nervous system, which lowers the rate of relapses.
- It is not approved for children or for primary progressive MS, and is started and monitored by a specialist experienced in MS care, although the tablets themselves are taken at home.
Side Effects
- The most common reactions, each affecting more than 1 in 10 people, are upper respiratory tract infection (about 37%), raised liver enzymes found on blood tests (about 23%), and high blood pressure (about 10%).
- Ponvory has no boxed warning, but it can lower heart rate and slow heart conduction, especially during the first days of dose titration, so report a very slow, fast, or irregular heartbeat, dizziness, or fainting straight away.
- It can also increase the risk of serious infection, so tell your healthcare provider about fever, chills, or unusual tiredness.
- Report yellowing of the skin or eyes, dark urine, or unexplained fatigue, which can point to liver injury, and any sudden change in vision, which can signal macular oedema.
- Seek urgent medical help for chest pain, severe breathlessness, or a very slow heartbeat, particularly in the first two weeks of treatment.
Precautions & Warnings
- Before starting Ponvory you will have a full blood count, ECG, liver function tests, an eye examination, and a review of your vaccination history, because the medicine can affect heart rhythm, liver enzymes, and infection risk.
- It must not be used within six months of a heart attack, unstable angina, stroke, TIA, or decompensated heart failure, or in people with Mobitz type II or higher AV block or sick sinus syndrome unless a pacemaker is fitted; the EU label also lists severe untreated infection, active cancer, and moderate-to-severe liver impairment as contraindications.
- Ponvory may cause fetal harm, and the EU label states it must not be used in pregnancy; women who could become pregnant need effective contraception during treatment and for one week after stopping, and must tell their provider immediately if pregnancy occurs.
- Liver enzymes and blood pressure are checked periodically, regular skin checks are advised because of a higher skin cancer risk with this drug class, and new breathing difficulty should be reported.
Avoid Interactions
- Do not receive live attenuated vaccines while taking Ponvory or for one to two weeks after stopping, as the response may be reduced or the infection may not be well controlled.
- Tell your healthcare provider about all other medicines, especially beta-blockers, diltiazem, verapamil, digoxin, or any other drug that slows the heart rate, since combining these with Ponvory increases the risk of a very slow heartbeat or conduction block, particularly during dose titration.
- Strong CYP3A4 and UGT1A1 inducers such as rifampicin are not recommended, as they can lower ponesimod levels and reduce its effect; avoid other QT-prolonging medicines and Class Ia or Class III antiarrhythmics where possible.
- Give your healthcare provider a complete list of prescription medicines, over-the-counter products, and supplements before starting Ponvory and before starting or stopping any other medicine while taking it.

Frequently Asked Questions
Ponvory (ponesimod) is a once-daily tablet used to treat relapsing forms of multiple sclerosis (MS) in adults. It lowers the number of immune cells reaching the brain and spinal cord, which reduces relapses. The EU approval is narrower, limited to relapsing MS with recent clinical or imaging activity.
Ponvory starts with a 14-day titration pack: one tablet daily, rising from 2mg to 9mg, to reduce heart rate effects. From day 15 you take a fixed 20mg tablet once daily, with or without food, swallowed whole.
More than 1 in 10 people get an upper respiratory infection, raised liver enzymes, or high blood pressure. Ponvory can also slow the heart rate, especially early in titration. Get urgent help for a very slow or irregular heartbeat, chest pain, or breathlessness.
Avoid live vaccines during treatment and for 1-2 weeks after stopping. Tell your provider about beta-blockers, verapamil, diltiazem, digoxin, or other heart-rate-lowering drugs, since combining them with ponesimod raises the risk of a very slow heartbeat.
If you miss more than 4 consecutive days of the 14-day starter pack or of maintenance dosing, you must restart the titration schedule from day 1 under medical supervision, because heart-rate effects can recur. Never resume at the 20mg dose after a long gap.
No. Ponvory may harm an unborn baby, and the EU label contraindicates it in pregnancy. Women who could become pregnant must use effective contraception while taking Ponvory and for one week after stopping, and tell their doctor at once if pregnancy occurs.
