Indications & Clinical Uses
- Prevnar 20 0.5 mL (pneumococcal 20-valent conjugate vaccine, Pfizer/Wyeth) is an injectable vaccine for active immunization against invasive disease caused by 20 serotypes of Streptococcus pneumoniae (1, 3, 4, 5, 6A, 6B, 7F, 8, 9V, 10A, 11A, 12F, 14, 15B, 18C, 19A, 19F, 22F, 23F, 33F), and for prevention of pneumonia from these serotypes in adults.
- It is approved for infants from 6 weeks of age through a four-dose series (including protection against middle-ear infection from seven shared serotypes), for children and adolescents through 17 years as a single dose, and for adults 18 years and older, including adults 60 and over, as a single dose.
- Some adult pneumonia indications for seven serotypes were granted under accelerated approval.
- Prevnar 20 is given only by a healthcare provider, by intramuscular injection.
- It is not interchangeable with Prevnar 13, an older, narrower-coverage vaccine from the same manufacturer that protects against 13 of these 20 serotypes only.
Side Effects
- In infants receiving the four-dose series, irritability (more than 6 in 10), injection-site pain and drowsiness (more than 3 in 10) are very common, with decreased appetite and redness occurring in more than 2 in 10.
- In children and adolescents given a single dose, injection-site pain (more than half) and drowsiness or fatigue (2 to 4 in 10) are most frequent.
- In adults, injection-site pain (more than 7 in 10 in younger adults), muscle pain, fatigue and headache (more than 3 in 10) are the leading reactions; older adults report somewhat lower rates.
- Prevnar 20 carries no boxed warning.
- Seek urgent medical help for swelling of the face or throat, breathing difficulty, wheezing or a fast heartbeat after vaccination, which can indicate a severe allergic reaction.
Precautions & Warnings
- Prevnar 20 must not be given after a severe allergic reaction to a previous dose, to any component, or to any diphtheria toxoid-containing vaccine, and epinephrine must be available when it is administered.
- Data in immunocompromised individuals are limited, and a reduced immune response is likely in this group, though vaccination is still generally recommended.
- Apnoea has been observed following intramuscular vaccination in some infants born prematurely, so monitoring for 48-72 hours is advised.
- Vaccination should usually be postponed during moderate or severe acute illness.
- Prevnar 20 must be given as a suspension for intramuscular injection only, never intravenously, and must not be mixed with any other vaccine or product in the same syringe.
Avoid Interactions
- Prevnar 20 may be co-administered with other age-appropriate vaccines, including influenza vaccine, at a separate injection site with a different syringe.
- Immunosuppressive therapies and conditions that weaken the immune system may reduce the antibody response, so timing of vaccination relative to such treatment should be discussed with the prescriber.
- Current guidance permits Prevnar 20 to be used as a single-dose alternative in some adults who would otherwise need both Prevnar 13 and a pneumococcal polysaccharide vaccine (PPSV23), but the two conjugate vaccines are not given together.
- There are no specific food, alcohol or herbal interactions.
- Tell the healthcare provider about all other vaccines and medicines before this vaccine is given.

Frequently Asked Questions
Prevnar 20 (pneumococcal 20-valent conjugate vaccine) protects against invasive disease and pneumonia caused by 20 strains of Streptococcus pneumoniae. It is given to infants from 6 weeks old, to children, and to adults 18 and over, including older adults. It is not given to infants under 6 weeks of age.
A healthcare provider gives Prevnar 20 as a single intramuscular injection into a muscle, never self-administered. Infants instead receive a four-dose series at 2, 4, 6 and 12-15 months, while older children and adults need only one dose. The exact schedule depends on the patient's age at the first dose.
Very common reactions include pain at the injection site, muscle pain, fatigue and headache in adults, and irritability or drowsiness in infants. Get urgent help for facial swelling, breathing difficulty or a fast heartbeat, which are signs of a severe allergic reaction.
Avoid Prevnar 20 after a severe allergic reaction to it, any of its ingredients, or any diphtheria-toxoid-containing vaccine. Tell the provider about any immune-weakening condition or treatment such as chemotherapy, since this can reduce how well the vaccine protects. This applies whether the reaction happens during the visit or hours afterward.
Yes, in many adults a single dose of Prevnar 20 can replace the older two-vaccine regimen of Prevnar 13 plus a separate pneumococcal polysaccharide vaccine. Ask your healthcare provider whether this simpler, single-vaccine option applies to your specific situation. This decision should always be made together with your healthcare provider.
No. Prevnar 20 covers the same 13 serotypes as Prevnar 13 plus seven more (8, 10A, 11A, 12F, 15B, 22F, 33F). The two vaccines are not interchangeable, so confirm which vaccine a patient previously received before a further dose. Confirming the correct vaccine avoids an incomplete or mismatched immunization record.
