Indications & Clinical Uses
- The Romazicon brand of flumazenil injection was discontinued from sale in the United States (confirmed via an FDA Federal Register notice dated 14 August 2014, which found the withdrawal was NOT for reasons of safety or effectiveness); generic flumazenil injection 0.1mg/mL remains FDA-approved and actively marketed and is clinically identical.
- Flumazenil is a benzodiazepine antagonist given by intravenous injection, used in adults and children aged 1 year and older to reverse the sedative effects of benzodiazepines after procedures done under conscious sedation or general anaesthesia, and in adults to help manage a known or suspected benzodiazepine overdose.
- It works by blocking benzodiazepine receptors in the brain, quickly reversing drowsiness and reduced breathing caused by these drugs, but it does not reverse the effects of opioids or other non-benzodiazepine sedatives.
- It is a hospital- or clinic-administered medicine given only by a healthcare provider trained in resuscitation, never a medicine patients keep or inject themselves.
Side Effects
- The most common effects of Romazicon (flumazenil) are nausea and vomiting (up to about 1 in 10 people), dizziness, injection-site pain, headache, abnormal or blurred vision, sweating, and feelings of agitation or anxiety as sedation wears off.
- Boxed warning: flumazenil has been associated with seizures, most often in people who have taken benzodiazepines long-term for sedation or in cases of mixed drug overdose, particularly with cyclic antidepressants.
- Resedation, breathing difficulty, tremor, and abnormal heartbeat can also occur once the reversal effect fades.
- Seek urgent medical attention for seizures, severe agitation, fainting, or return of sedation and slowed breathing after leaving the clinic.
Precautions & Warnings
- Romazicon must be given by intravenous injection only, by a healthcare provider with resuscitation equipment on hand, because sedation, low oxygen levels or seizures can return once its effect (about 30-60 minutes) wears off before the benzodiazepine has cleared.
- It should not be used if benzodiazepines are being given to control a life-threatening condition such as raised pressure in the brain or status epilepticus, in known hypersensitivity to flumazenil or benzodiazepines, or where a cyclic antidepressant overdose is suspected, since seizure risk rises sharply.
- Extra caution is needed in head injury, liver impairment, benzodiazepine dependence, panic disorder, and pregnancy or breastfeeding, where use should be limited to situations where the benefit clearly outweighs the risk.
- Patients must not drive or operate machinery for at least 18-24 hours afterward.
Avoid Interactions
- Romazicon specifically reverses benzodiazepines and does not counteract opioids, alcohol, barbiturates, or other sedative-hypnotics, so residual sedation from these can persist unrecognised after it is given.
- Its use in a mixed overdose that includes cyclic antidepressants (e.g. amitriptyline) can provoke seizures, because unmasking benzodiazepine reversal removes their anticonvulsant protection.
- Flumazenil does not appear to interact directly with narcotics, inhalational anaesthetics or muscle relaxants used during surgery, but the healthcare team must know about every medicine, illicit drug or supplement the patient has taken, including chronic benzodiazepine therapy, since abrupt reversal in dependent patients can trigger a withdrawal-type reaction.
- Do not mix Romazicon in the same syringe with other drugs.

Frequently Asked Questions
The Romazicon brand was discontinued in the US in 2014, but not for safety reasons. Generic flumazenil injection is still FDA-approved and available, and works the same way to reverse benzodiazepine sedation after procedures or in suspected benzodiazepine overdose, given only by a healthcare provider.
A healthcare provider gives Romazicon as a slow intravenous injection, usually in small repeated doses over 1-2 minutes until the desired reversal is reached. It is never self-injected or taken by mouth. You stay monitored afterward because its effect can wear off before the benzodiazepine does.
Common Romazicon side effects include nausea, dizziness, headache, injection-site pain and agitation. Its boxed warning is seizure risk, especially with long-term benzodiazepine use or mixed overdose involving antidepressants. Seek urgent help for seizures, fainting, or sedation and slow breathing returning after you leave the clinic.
Do not drive, operate machinery, or make important decisions for at least 18-24 hours after Romazicon, since sedation can return once its effect fades. Avoid alcohol and other sedatives during this time. Tell your healthcare provider about all medicines you take, especially antidepressants and long-term benzodiazepine use.
You are monitored in the clinic or hospital for return of sedation, slowed breathing, or seizures, because Romazicon's effect (roughly 30-60 minutes) can be shorter than the benzodiazepine it reverses. Vital signs and alertness are checked before you are discharged, usually with someone to accompany you home.
Yes, Romazicon is approved to reverse conscious sedation in children aged 1 year and older, given by a healthcare provider with dosing adjusted to body weight. Its use for suspected benzodiazepine overdose is approved for adults. Pregnant or breastfeeding patients should discuss risks and benefits with their provider first.
