Indications & Clinical Uses
- Rybelsus 3 mg tablets (semaglutide, an oral glucagon-like peptide-1 receptor agonist) manufactured by Novo Nordisk are approved as an adjunct to diet and exercise to improve glycemic control in adults with type 2 diabetes mellitus, and, since October 2025, also to reduce the risk of major adverse cardiovascular events (cardiovascular death, non-fatal heart attack, or non-fatal stroke) in adults with type 2 diabetes who are at high cardiovascular risk.
- The 3 mg strength is a starting/titration dose; treatment is usually increased to 7 mg and then 14 mg once daily as tolerated.
- Rybelsus is not approved for type 1 diabetes or diabetic ketoacidosis, and it is not interchangeable with injectable semaglutide products such as Ozempic or Wegovy, which use different strengths and a different route of administration.
Side Effects
- Rybelsus carries a boxed warning: in rodent studies, semaglutide caused thyroid C-cell tumors, including medullary thyroid carcinoma; it is unknown whether this occurs in humans, but Rybelsus is contraindicated in people with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2.
- Very common side effects (more than 1 in 10 people) include nausea, abdominal pain, diarrhea, decreased appetite, vomiting and constipation, especially during dose increases.
- Report a lump or swelling in the neck, hoarseness, trouble swallowing, severe or persistent abdominal pain (possible pancreatitis), vision changes, signs of low blood sugar such as shakiness or confusion (especially when combined with insulin or sulfonylureas), or a severe allergic reaction promptly.
Precautions & Warnings
- Do not use Rybelsus if you or a family member have had medullary thyroid carcinoma or MEN 2, or if you have had a serious hypersensitivity reaction to semaglutide.
- Tell your healthcare provider about any history of pancreatitis, diabetic retinopathy, gallbladder disease, or kidney problems, since dehydration from vomiting or diarrhea can worsen kidney function.
- Take Rybelsus first thing in the morning on an empty stomach with no more than 4 ounces of plain water, then wait at least 30 minutes before eating, drinking anything else, or taking other oral medicines, because food and other liquids reduce its absorption.
- Tell your surgical or anesthesia team you take Rybelsus before any procedure, since delayed stomach emptying can increase the risk of aspiration.
- Discuss pregnancy planning, as it is generally not recommended during pregnancy.
Avoid Interactions
- Rybelsus slows stomach emptying and can affect the absorption of other oral medicines taken at the same time, so it must be taken separately as directed and other oral medicines should generally be taken after the 30-minute waiting period, or as advised by your prescriber.
- It increases exposure to oral levothyroxine, so thyroid function should be monitored if you take thyroid hormone replacement.
- When used together with insulin or sulfonylureas (such as glipizide or glimepiride), the risk of low blood sugar (hypoglycemia) rises, and a dose reduction of the insulin or sulfonylurea may be needed.
- Tell your healthcare provider about all other diabetes medicines, warfarin, and any other prescription, over-the-counter or herbal products you take.

Frequently Asked Questions
Rybelsus (semaglutide) improves blood sugar control in adults with type 2 diabetes, alongside diet and exercise. Since October 2025 it is also approved to reduce the risk of heart attack, stroke or cardiovascular death in adults with type 2 diabetes at high cardiovascular risk. The 3 mg dose is a starting strength.
Take Rybelsus once daily first thing in the morning, on an empty stomach, with no more than 4 ounces of plain water. Wait at least 30 minutes before eating, drinking anything else, or taking other oral medicines, since food reduces how much semaglutide your body absorbs.
Common Rybelsus side effects include nausea, stomach pain, diarrhea, reduced appetite and vomiting, especially when the dose is increased. It carries a boxed warning for thyroid C-cell tumors seen in animal studies; report any neck lump, hoarseness or trouble swallowing to your provider promptly.
Avoid eating, drinking other liquids, or taking other oral medicines within 30 minutes of your Rybelsus dose, since this reduces absorption. Combining it with insulin or sulfonylureas raises low blood sugar risk. Tell your provider about all medicines, since Rybelsus increases levothyroxine levels.
Do not take Rybelsus if you or a family member have had medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2, or if you have had a serious allergic reaction to semaglutide. This is a contraindication tied to the boxed warning on thyroid tumors seen in animal studies.
Rybelsus is generally not recommended during pregnancy; discuss pregnancy plans with your healthcare provider before starting or continuing it. It is approved only for adults with type 2 diabetes, not for type 1 diabetes, and its use in children has not been established.
