Indications & Clinical Uses
- Rytary extended-release capsules combine carbidopa and levodopa, in this strength 61.25 mg/245 mg per capsule, one of four available strengths (23.75 mg/95 mg, 36.25 mg/145 mg, 48.75 mg/195 mg and 61.25 mg/245 mg).
- Levodopa restores dopamine in the brain and carbidopa reduces its breakdown outside the brain; the extended-release formulation is designed to give more consistent levodopa levels than immediate-release carbidopa-levodopa.
- Rytary is approved for adults with Parkinson's disease, post-encephalitic parkinsonism, and parkinsonism that may follow carbon monoxide or manganese poisoning.
- Because Rytary releases levodopa over an extended period, its dose and frequency are not interchangeable milligram-for-milligram with immediate-release carbidopa-levodopa products, and switching between them requires a healthcare provider to recalculate the regimen.
- Rytary is taken by mouth at home; it is not given by injection or infusion.
Side Effects
- In earlier-stage Parkinson's disease, the most common Rytary side effects include nausea, dizziness, headache, insomnia and involuntary movements (dyskinesia).
- In more advanced disease, nausea and headache are most frequently reported.
- Rytary carries no boxed warning, but the label highlights an important risk: sudden discontinuation or a rapid dose reduction can cause a syndrome resembling neuroleptic malignant syndrome, with high fever, muscle rigidity, confusion and changes in blood pressure and heart rate, so treatment must be tapered rather than stopped abruptly except under medical supervision.
- Some patients experience sudden episodes of falling asleep during daily activities, including driving, sometimes without warning; report this to your healthcare provider immediately.
- Hallucinations, unusual urges and low blood pressure on standing can also occur and should be reported.
Precautions & Warnings
- Rytary must not be used with nonselective monoamine oxidase inhibitors (MAOIs), which should be stopped at least two weeks before starting treatment, because the combination can cause a dangerous rise in blood pressure.
- Use with caution in patients with a history of melanoma, psychiatric illness, cardiovascular disease, or asthma, and in those with glaucoma.
- Blood pressure, mental state and skin should be monitored periodically, with regular skin checks for melanoma given a reported association with Parkinson's disease and levodopa therapy.
- Rytary capsules should be swallowed whole or, if needed, opened with the contents sprinkled on a small amount of soft food and taken immediately without chewing; do not crush or chew the beads.
- Avoid driving or operating machinery until you know how Rytary affects your alertness, and never stop Rytary suddenly without medical advice.
Avoid Interactions
- Rytary must not be combined with nonselective MAO inhibitors; selective MAO-B inhibitors such as selegiline or rasagiline may be used with caution and monitoring for low blood pressure on standing.
- High-protein meals can reduce levodopa absorption, so taking Rytary consistently in relation to meals helps maintain a steady effect.
- Iron salts and iron-containing supplements can reduce levodopa absorption and should be spaced apart from Rytary dosing.
- Antipsychotic medicines and other dopamine-blocking drugs, such as metoclopramide, can reduce Rytary's effectiveness and worsen Parkinson's symptoms.
- Tell your healthcare provider and pharmacist about all other medicines and supplements you take, including over-the-counter products and iron supplements, before starting Rytary.

Frequently Asked Questions
Rytary (carbidopa and levodopa extended-release capsules) treats Parkinson's disease and parkinsonism following encephalitis, carbon monoxide poisoning or manganese poisoning in adults. Levodopa restores brain dopamine, while carbidopa reduces side effects, and the extended-release design gives steadier symptom control than immediate-release versions.
Rytary capsules are taken by mouth, usually three times daily to start, and can be swallowed whole or opened and sprinkled on soft food. Do not crush or chew the beads inside. Your healthcare provider adjusts your dose and frequency based on your response.
Common Rytary side effects include nausea, dizziness, headache, insomnia and involuntary movements (dyskinesia). Some people suddenly fall asleep during daily activities without warning - avoid driving until you know how Rytary affects you. Report hallucinations, fainting or unusual urges to your healthcare provider.
Avoid nonselective MAO inhibitors, which must be stopped at least 2 weeks before starting Rytary. High-protein meals and iron supplements can reduce levodopa absorption, so space them apart from doses. Never stop Rytary suddenly - this can cause dangerous fever and muscle rigidity.
Take a missed Rytary dose as soon as you remember, unless it is nearly time for the next one - then skip it and continue your normal schedule. Do not take a double dose. Never stop Rytary abruptly, as sudden discontinuation can trigger severe fever and muscle stiffness.
Not on a simple milligram-for-milligram basis. Rytary releases levodopa over an extended period, so its dosing differs from immediate-release carbidopa-levodopa. Your healthcare provider must recalculate your dose and schedule if you switch between these products.
