Indications & Clinical Uses
- Serevent Diskus 50 mcg contains salmeterol xinafoate, a long-acting beta2-adrenergic agonist (LABA) inhaled as a dry powder.
- It is approved for the long-term, twice-daily maintenance treatment of asthma in patients aged 4 years and older, always used together with an inhaled corticosteroid, for prevention of exercise-induced bronchospasm in patients 4 years and older, and for maintenance treatment of bronchospasm associated with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema.
- Serevent Diskus is not a rescue inhaler and must not be used to relieve a sudden asthma or COPD attack; a fast-acting bronchodilator is needed for that.
- Serevent Diskus is self-administered at home using the Diskus inhaler device, one inhalation approximately every 12 hours.
Side Effects
- Serevent Diskus carries a boxed warning: long-acting beta agonists such as salmeterol increase the risk of asthma-related death when used as monotherapy without an inhaled corticosteroid, and a large US trial found more deaths in patients taking salmeterol alone than placebo.
- Common side effects include headache, flu-like symptoms, throat irritation or pharyngitis, nasal and sinus congestion, and, in COPD, cough and musculoskeletal pain.
- Serevent Diskus can rarely cause paradoxical bronchospasm immediately after inhalation, which requires switching to another bronchodilator and stopping Serevent Diskus; it can also cause a fast heart rate, tremor, or low blood potassium.
- Seek urgent medical help for worsening wheeze right after using the inhaler, chest pain, a racing heartbeat, or a severe asthma attack that doesn't respond to your reliever inhaler.
Precautions & Warnings
- Serevent Diskus must always be used together with an inhaled corticosteroid for asthma, never as the only asthma treatment, and it is not a substitute for a corticosteroid or for a fast-acting rescue inhaler, which should always be carried for sudden symptoms.
- Do not start Serevent Diskus during a period of rapidly worsening or acutely deteriorating asthma or COPD.
- Use cautiously if you have heart disease, an irregular heart rhythm, high blood pressure, seizures, an overactive thyroid, or diabetes, since salmeterol can affect heart rate and blood sugar.
- Avoid using other long-acting beta-agonist medicines at the same time as Serevent Diskus.
- Keep the Diskus in its foil pouch until first use, and discard it six weeks after opening the pouch or when the dose counter reads zero, whichever comes first.
Avoid Interactions
- Tell your healthcare provider about all medicines you take before using Serevent Diskus.
- Strong CYP3A4 inhibitors, such as ketoconazole and ritonavir, can raise salmeterol levels and increase the risk of QT prolongation and heart-related side effects.
- Beta-blockers, including eye drops, may block salmeterol's effect and can cause severe bronchospasm in people with asthma; use them together only if necessary and with caution.
- Monoamine oxidase inhibitors and tricyclic antidepressants can increase salmeterol's effects on the heart and blood vessels.
- Diuretics that lower potassium, such as loop or thiazide diuretics, can worsen the low potassium and ECG changes that salmeterol can cause.
- Always tell your prescriber about every other medicine and supplement before starting or continuing Serevent Diskus.

Frequently Asked Questions
Serevent Diskus (salmeterol) is a twice-daily inhaler that helps control asthma when used with an inhaled corticosteroid, prevents exercise-induced breathing problems, and eases COPD-related bronchospasm. It's a long-term controller, not a rescue inhaler, so it won't stop a sudden asthma attack.
Inhale one dose from the Serevent Diskus device about every 12 hours, roughly 12 hours apart. Use the built-in dose counter to track doses left, and keep the device dry. Discard it 6 weeks after opening the foil pouch, or when the counter reads zero, whichever comes first.
Serevent Diskus carries a boxed warning for increased risk of asthma-related death when salmeterol is used alone, without an inhaled corticosteroid. Common side effects are headache, throat irritation and sinus congestion. Get urgent help for sudden worsening wheeze right after inhaling, or chest pain.
Never use Serevent Diskus as your only asthma treatment -- always pair it with an inhaled corticosteroid, and always carry a fast-acting rescue inhaler too. Avoid other long-acting beta-agonists, and tell your healthcare provider about beta-blockers, MAOIs, or CYP3A4 inhibitors like ketoconazole.
If you miss a Serevent Diskus dose, take it when you remember and resume your normal twice-daily schedule; don't double up. Never use extra doses to relieve sudden symptoms -- use your rescue inhaler instead, and call your healthcare provider if you're using it more often.
Yes -- Serevent Diskus is approved for asthma maintenance and exercise-induced bronchospasm prevention in children aged 4 and older, always alongside an inhaled corticosteroid. It's not approved for younger children. Talk to your child's healthcare provider about the boxed warning before starting treatment.
