Indications & Clinical Uses

  • Sinemet 25mg/100mg (carbidopa 25 mg / levodopa 100 mg) tablets combine a dopamine precursor, levodopa, with the peripheral decarboxylase inhibitor carbidopa, which reduces the levodopa dose needed and limits nausea.
  • Sinemet is indicated for the treatment of idiopathic Parkinson's disease, post-encephalitic parkinsonism, and symptomatic parkinsonism that can follow carbon monoxide or manganese intoxication, in adults.
  • Carbidopa alone has no therapeutic effect; combining it with levodopa allows more levodopa to reach the brain and be converted to dopamine, improving rigidity, tremor and slowness of movement.
  • Sinemet is not indicated for drug-induced extrapyramidal reactions.
  • It is taken by mouth, usually several times a day, and the dose is adjusted individually to the lowest effective amount.
  • The same active combination is also marketed generically as carbidopa/levodopa and in extended-release and orally disintegrating forms under related brand names.

Side Effects

  • The most frequently reported reactions with Sinemet are involuntary movements (dyskinesias) and nausea; other common effects include dizziness, drowsiness, dry mouth, and harmless reddish discolouration of urine or sweat.
  • Sinemet does not carry a boxed warning, but the label warns that some people fall asleep suddenly during normal daily activities, sometimes without feeling drowsy beforehand, which can be dangerous while driving.
  • Report new or worsening hallucinations, confusion, severe dizziness on standing, uncontrollable movements, high fever with muscle rigidity and confusion (which can signal a rare but serious reaction after abrupt withdrawal), or any new skin lesions, since Parkinson's patients have a higher risk of melanoma.
  • Seek urgent care for chest pain, irregular heartbeat, or severe agitation.

Precautions & Warnings

  • Sinemet is contraindicated with nonselective monoamine oxidase (MAO) inhibitors, which must be stopped at least two weeks beforehand, and in narrow-angle glaucoma or known hypersensitivity to either component.
  • It is used with caution in people with a history of psychosis, cardiovascular or lung disease, peptic ulcer, or seizures, and in those with asthma or chronic bronchitis.
  • Routine monitoring of hepatic, haematologic, cardiovascular and renal function is recommended during extended therapy, along with periodic skin checks for melanoma.
  • Impulse-control problems such as gambling, hypersexuality or compulsive shopping have been reported.
  • Vitamin B6 supplements can reduce levodopa's effect.
  • Do not stop Sinemet abruptly; dose reduction should be gradual under medical supervision.
  • Warn patients about the risk of sudden sleep onset before driving or operating machinery.

Avoid Interactions

  • Tell your healthcare provider about every medicine, herbal product and supplement you take.
  • Do not combine Sinemet with nonselective MAO inhibitors such as phenelzine; a two-week washout is required first.
  • Iron salts and high-protein meals can reduce carbidopa/levodopa absorption, so timing away from iron supplements and large protein loads helps.
  • Antipsychotics and other dopamine-receptor blockers (such as metoclopramide) can reduce Sinemet's effect and worsen parkinsonism.
  • Antihypertensive medicines may cause additive low blood pressure on standing.
  • Isoniazid can reduce levodopa's effect.
  • Dopamine-depleting agents and certain anaesthetics used with tricyclic antidepressants have caused blood pressure swings.
  • Do not take other levodopa-containing products together with Sinemet.
