Indications & Clinical Uses
- Striverdi Respimat 2.5 mcg (olodaterol), a long-acting beta2-adrenergic agonist (LABA) bronchodilator delivered via the Respimat soft-mist inhaler, is approved for the long-term, once-daily maintenance treatment of airflow obstruction in adults with chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema.
- Each actuation releases 2.5 mcg olodaterol and the approved dose is two actuations (5 mcg total) once daily, inhaled at the same time every day.
- Striverdi Respimat is not a rescue inhaler for sudden breathlessness and is not approved for asthma; in asthma, a LABA must never be used without an inhaled corticosteroid.
- It is marketed by Boehringer Ingelheim in the United States, the European Union and other markets under the same brand name, and is self-administered by the patient at home rather than given by a healthcare provider.
Side Effects
- The most commonly reported reactions with Striverdi Respimat in clinical trials, occurring in 2% or more of patients and more often than with placebo, are nasopharyngitis (11%), upper respiratory tract infection (8%), bronchitis, cough, back pain, diarrhoea, urinary tract infection, dizziness, rash and joint pain.
- Striverdi Respimat carries no boxed warning, but like other long-acting beta agonists it can cause paradoxical bronchospasm — stop use immediately and seek urgent care if breathing suddenly worsens after a dose — and excessive use may cause significant cardiovascular effects such as a fast heart rate, blood pressure changes, tremor or palpitations.
- Report any worsening cough, chest tightness, irregular heartbeat or allergic reaction (swelling, rash, difficulty breathing) to your healthcare provider promptly, and have your inhaler technique checked regularly.
Precautions & Warnings
- Striverdi Respimat must not be used to treat acute, rapidly worsening COPD symptoms or as a rescue inhaler, and it is not approved for asthma; a LABA used without an inhaled corticosteroid is contraindicated in asthma because it increases the risk of asthma-related death.
- Use with caution if you have heart rhythm problems, high blood pressure, a seizure disorder, an overactive thyroid, or diabetes, since beta-agonists can affect heart rate, blood pressure, blood sugar and potassium levels.
- Tell your healthcare provider if you are pregnant, planning pregnancy or breastfeeding, as effects on the fetus and infant are not fully established.
- There are no specific driving restrictions, but stop treatment and seek medical advice if breathing, chest pain or heartbeat suddenly worsen.
- Do not exceed the prescribed once-daily dose of two actuations.
Avoid Interactions
- Tell your healthcare provider about all medicines you take.
- Other adrenergic (sympathomimetic) drugs and stimulants may add to Striverdi Respimat's cardiovascular effects and should be used with caution.
- Non-potassium-sparing diuretics such as thiazides or loop diuretics can worsen ECG changes and low potassium caused by beta-agonists; ECG and potassium monitoring may be needed.
- Monoamine oxidase inhibitors, tricyclic antidepressants and other drugs that prolong the QT interval should be used with extreme caution because they can add to cardiovascular effects.
- Beta-blockers, including eye drops, may reduce Striverdi Respimat's effectiveness and can provoke severe bronchospasm; a cardioselective beta-blocker is preferred if one is needed.
- Always disclose supplements and over-the-counter cold or asthma remedies, since extra stimulant ingredients can add to side effects.

Frequently Asked Questions
Striverdi Respimat (olodaterol) is a long-acting bronchodilator used for once-daily maintenance treatment of chronic obstructive pulmonary disease (COPD), including chronic bronchitis and emphysema, in adults. It is not a rescue inhaler for sudden breathlessness and is not approved for asthma.
You inhale two actuations from the Respimat device once daily, at the same time each day, giving a total dose of 5 mcg olodaterol. Prime the inhaler before first use and never take more than two actuations in 24 hours.
Common effects include nasopharyngitis, upper respiratory infection, bronchitis, cough, back pain and dizziness. Seek urgent care if breathing suddenly worsens after a dose (paradoxical bronchospasm), or if you get chest pain, a fast or irregular heartbeat, or swelling and rash.
Avoid using it for asthma without an inhaled corticosteroid, since LABAs increase the risk of asthma-related death in that setting. Tell your provider about other stimulant medicines, diuretics, MAO inhibitors and beta-blockers, which can change how olodaterol affects your heart.
Take the missed dose as soon as you remember, then continue your usual once-daily schedule as before. Never take a double dose, or more than two actuations total, within the same 24-hour period. Store the inhaler at 20°C to 25°C, away from moisture.
Striverdi Respimat is intended for ongoing, long-term daily COPD control, not short-term or occasional use. Tell your healthcare provider if you are pregnant, planning a pregnancy or breastfeeding, since effects on the unborn baby or infant are not fully established and safety data remain limited.
