Indications & Clinical Uses

  • Tirosint 100 mcg (levothyroxine sodium) soft-gel capsules are a synthetic form of thyroxine (T4), the hormone the thyroid gland normally makes, made by IBSA.
  • Tirosint is indicated as replacement therapy for hypothyroidism in adults and in children aged 6 years and older, and as an adjunct to surgery and radioiodine therapy to suppress TSH in patients with well-differentiated thyroid cancer.
  • Unlike standard levothyroxine tablets, its liquid-gel capsule is formulated without gluten, lactose or dyes, which some patients with absorption problems or excipient sensitivity may find easier to tolerate; it does not change the drug's fundamental hormone-replacement purpose.
  • It is taken by mouth once daily at home, not given by a clinician.

Side Effects

  • Tirosint carries a boxed warning: thyroid hormones, including levothyroxine, must not be used for weight loss or to treat obesity, and doses beyond what a person needs for hormone replacement can cause serious, potentially fatal effects on the heart, especially when combined with appetite-suppressant medicines.
  • Side effects usually reflect too much thyroid hormone from an excessive dose: palpitations, a fast heartbeat, tremor, nervousness, trouble sleeping, unexplained weight loss, diarrhoea and heat intolerance.
  • Report a racing or irregular heartbeat, chest pain, or severe anxiety promptly, since these suggest the dose needs adjusting.

Precautions & Warnings

  • Tirosint's only contraindication is uncorrected adrenal insufficiency, which must be treated first, or a sudden dose can trigger an acute adrenal crisis.
  • Take it on an empty stomach, 30 to 60 minutes before breakfast, at the same time every day, and keep the timing consistent since food and gaps around dosing change absorption.
  • Blood tests (TSH, sometimes free T4) are used to check and adjust the dose, typically 6 to 8 weeks after any change.
  • Use caution in older people and those with heart disease, who often start on a lower dose.
  • Tell your doctor if you become pregnant, since thyroid hormone needs usually rise during pregnancy and monitoring should be more frequent.

Avoid Interactions

  • Tell your healthcare provider and pharmacist about all medicines and supplements you take.
  • Calcium carbonate, iron supplements, bile acid sequestrants such as cholestyramine, proton pump inhibitors, sucralfate, and antacids containing aluminium or magnesium can all reduce how much levothyroxine you absorb, so separate them from Tirosint by at least four hours.
  • Tirosint can increase the effect of warfarin, so clotting tests may need closer monitoring, and it can change insulin or oral diabetes medicine requirements.
  • Oestrogen-containing contraceptives can raise the amount of thyroid-binding protein in the blood, sometimes requiring a Tirosint dose adjustment.
