Indications & Clinical Uses

  • Verzenio 150 mg tablets contain abemaciclib, a kinase (CDK4/6) inhibitor used in adults with hormone receptor-positive (HR+), HER2-negative breast cancer.
  • In the US it is approved for adjuvant treatment, together with endocrine therapy, of node-positive early breast cancer at high risk of recurrence; for advanced or metastatic disease as initial endocrine-based therapy with an aromatase inhibitor; with fulvestrant after disease progression on endocrine therapy; and as a single agent in metastatic disease after prior endocrine therapy and chemotherapy.
  • It is prescribed and supervised by a cancer specialist and taken at home.
  • The same medicine is marketed as Verzenios in the EU and UK.
  • The current US label (revised February 2025) is in use and no withdrawal or discontinuation was found.
  • Verzenio tablets come in 50, 100, 150 and 200 mg strengths, which are used for dose adjustments.

Side Effects

  • Very common reactions (20% or more of patients) are diarrhea, low white blood cell counts (neutropenia and leukopenia), nausea, abdominal pain, infections, tiredness, anemia, decreased appetite, vomiting, headache, hair loss and low platelets.
  • Diarrhea affects roughly 81% to 90% of patients, usually within the first month, and can be severe.
  • Serious reactions include severe neutropenia, lung inflammation (interstitial lung disease or pneumonitis, about 3%), liver problems (raised ALT and AST) and blood clots in veins or lungs (2% to 5%).
  • Start anti-diarrheal medicine at the first sign of loose stools and drink plenty of fluids, and contact your care team promptly about fever, new cough or breathlessness, or leg swelling.
  • There is no boxed warning.

Precautions & Warnings

  • Blood counts are checked before treatment, every 2 weeks for the first 2 months, then monthly for 2 months and as clinically needed; liver tests follow the same schedule.
  • Verzenio can cause fetal harm: women who can become pregnant should use effective contraception during treatment and for 3 weeks after the last dose and should not breastfeed during that time, and it may impair fertility in men.
  • Severe hepatic impairment requires once-daily dosing, and use in severe renal impairment has not been studied.
  • If you vomit or miss a dose, take the next dose at the usual time and do not take an extra dose.
  • Swallow tablets whole; do not chew, crush or split them.

Avoid Interactions

  • Increasing exposure or toxicity: avoid ketoconazole; with other strong CYP3A inhibitors (for example clarithromycin or itraconazole) the dose is reduced to 100 mg twice daily; moderate CYP3A inhibitors need monitoring and possible dose reduction; avoid grapefruit and grapefruit juice.
  • Reducing effectiveness: avoid strong and moderate CYP3A inducers such as rifampin and carbamazepine.
  • Tell your care team about every prescription and over-the-counter medicine, herbal product and supplement you take, and do not start or stop any of them without checking first, because starting or stopping a CYP3A inhibitor or inducer can change how much abemaciclib is in your body and your dose may need to be reviewed.
