Indications & Clinical Uses

  • Quadracel is a diphtheria and tetanus toxoids and acellular pertussis adsorbed and inactivated poliovirus (DTaP-IPV) vaccine supplied as a single 0.5 mL dose for intramuscular injection, manufactured by Sanofi Pasteur Limited in Toronto, Canada and distributed in the US by Sanofi Pasteur Inc.
  • It is FDA-approved for active immunisation of children 4 through 6 years of age, given as the fifth dose of the DTaP series and the fourth or fifth dose of the IPV series, in children whose earlier doses used Pentacel, Daptacel and/or Vaxelis vaccines.
  • Quadracel is given only by a healthcare professional in a clinic or pharmacy setting -- it is never self-administered and there is no dose to remember at home.
  • It is not interchangeable with other manufacturers' DTaP-IPV products for completing the same series unless the original product is unavailable.

Side Effects

  • Injection-site reactions are very common: pain in more than 3 in 4 children, swelling in more than 2 in 3, and redness in more than half.
  • Systemic reactions are also common, including muscle aches in more than half, tiredness in more than 1 in 3, and headache in more than 1 in 6; fever is less frequent.
  • Quadracel's label carries no boxed warning.
  • Rare but serious reactions include severe allergic reaction (anaphylaxis), which is why children are usually observed for 15 minutes after vaccination, and fainting (syncope), which can happen after any injection, particularly in older children and adolescents.
  • Seek urgent medical attention for difficulty breathing, facial or throat swelling, hives, a high fever, unusual drowsiness, or a seizure following vaccination.

Precautions & Warnings

  • Quadracel must not be given to a child who has had a severe allergic reaction to a previous dose or to any vaccine component, or who developed encephalopathy within 7 days of an earlier pertussis-containing vaccine.
  • Vaccination should be postponed, not skipped, during moderate or severe acute illness, but can proceed with a mild illness such as a cold.
  • Use caution in children with a history of Guillain-Barre syndrome or a progressive neurological disorder, which should be stabilised before immunisation.
  • Quadracel should be stored at 2 to 8C and never frozen; a vaccine that has been frozen must be discarded.
  • Tell the vaccinator about any bleeding disorder, as intramuscular injection requires care in these children, and about any immune-suppressing condition or treatment, which may reduce the vaccine's effectiveness.

Avoid Interactions

  • Quadracel can be given at the same visit as other routine childhood vaccines, such as MMR, varicella or influenza vaccine, but each vaccine must be given with a separate syringe at a different injection site.
  • Children receiving immunosuppressive therapy (such as high-dose corticosteroids, chemotherapy or radiation) or who have an immunodeficiency may have a reduced immune response to Quadracel, and vaccination timing should be discussed with the healthcare provider.
  • There are no known drug interactions requiring dose adjustment, but tell the vaccinator about all medicines, supplements and any recent or planned vaccinations, and about any allergic reaction to a previous vaccine.
