Indications & Clinical Uses
- Micardis 40 mg (telmisartan) is an angiotensin II receptor blocker (ARB) approved to treat high blood pressure (hypertension) in adults, either alone or with other antihypertensive medicines, to reduce the risk of stroke, heart attack, and death from cardiovascular causes.
- It is also approved in the US to reduce the risk of heart attack, stroke, or death from cardiovascular disease in patients aged 55 and older at high cardiovascular risk (such as coronary artery disease, peripheral arterial disease, stroke, or high-risk diabetes) who cannot take ACE inhibitors; this second use is not approved in the EU, where telmisartan is licensed for hypertension only.
- Telmisartan is also sold combined with hydrochlorothiazide as Micardis HCT (US) or MicardisPlus (EU/international), a different, higher-strength combination product not interchangeable with Micardis alone.
Side Effects
- Micardis carries a boxed warning: stop taking it as soon as pregnancy is detected, because drugs that act on the renin-angiotensin system can cause injury and death to the developing baby.
- Common side effects in clinical trials included upper respiratory tract infection (about 7%), diarrhea, sinusitis, and back pain, each affecting around 3% of patients; dizziness can also occur, especially after the first dose or in patients using other blood-pressure medicines.
- Serious but less common effects include a sharp drop in blood pressure, kidney function changes, high potassium levels, and rare allergic swelling of the face, lips, tongue, or throat (angioedema).
- Seek emergency care immediately for swelling of the face or throat, or difficulty breathing or swallowing.
Precautions & Warnings
- Tell your healthcare provider immediately if you become pregnant or are planning pregnancy; Micardis must be stopped as soon as pregnancy is confirmed because of the risk of fetal harm, and it is not recommended during breastfeeding.
- Blood pressure, kidney function, and potassium levels should be checked periodically, especially in people with kidney or liver impairment, heart failure, or those on a salt-restricted diet, as these increase the risk of low blood pressure or high potassium.
- Volume- or salt-depleted patients should have this corrected before starting treatment.
- Micardis is not recommended with aliskiren in patients with diabetes or moderate-to-severe kidney impairment.
- It may cause dizziness, so use caution when driving or operating machinery until you know how the medicine affects you.
- Use in severe liver impairment is not recommended.
Avoid Interactions
- Tell your healthcare provider about all medicines you take, especially other blood pressure medicines, potassium supplements or salt substitutes, and NSAIDs (such as ibuprofen or naproxen), which can reduce Micardis's blood-pressure-lowering effect and increase the risk of kidney problems, particularly in older adults or those who are dehydrated.
- Combining Micardis with ACE inhibitors or aliskiren increases the risk of low blood pressure, high potassium, and kidney impairment, and is not recommended, especially in people with diabetes or kidney disease.
- Digoxin blood levels can rise when taken with Micardis, so monitoring may be needed.
- Lithium levels can increase with ARBs, raising the risk of toxicity, so lithium levels should be monitored closely if the two are used together.

Frequently Asked Questions
Micardis 40 mg (telmisartan) is an ARB used to treat high blood pressure in adults, alone or with other medicines. In some patients aged 55 and over at high cardiovascular risk who cannot take ACE inhibitors, it also lowers the risk of heart attack, stroke, and cardiovascular death.
Take one Micardis tablet by mouth once daily, with or without food, at about the same time each day. Swallow it with water and keep tablets in the blister pack until just before use. Your healthcare provider may adjust your dose based on your blood pressure response.
Common side effects include upper respiratory infection, diarrhea, sinusitis, back pain, and dizziness. Micardis carries a boxed warning: it must be stopped as soon as pregnancy is detected, because it can seriously harm an unborn baby. Get emergency help for facial or throat swelling or breathing trouble.
Avoid NSAIDs like ibuprofen, which can reduce Micardis's effect and stress the kidneys, and potassium supplements or salt substitutes, which can raise potassium too high. Don't combine Micardis with aliskiren if you have diabetes or kidney disease. Tell your provider about all other medicines, including lithium.
Your healthcare provider will check your blood pressure, kidney function, and blood potassium periodically, especially when starting treatment or adjusting the dose. This is more important if you have kidney or liver problems, heart failure, or are on a low-salt diet or diuretics.
No. Micardis must be stopped as soon as pregnancy is confirmed, because ARBs can cause serious injury or death to the developing baby, especially in the second and third trimesters. Tell your healthcare provider right away if you are pregnant or planning to become pregnant.
