Indications & Clinical Uses

  • Plegridy contains peginterferon beta-1a, a pegylated form of interferon beta, used to treat relapsing forms of multiple sclerosis (MS) in adults, including clinically isolated syndrome, relapsing-remitting MS, and active secondary progressive MS with relapses.
  • Plegridy 125 mcg is given as a subcutaneous injection every 14 days once the starting-dose titration is complete — an initial lower-dose starter pack of 63 mcg and 94 mcg pens is used for the first month to reduce flu-like reactions — and in some markets it may also be given intramuscularly under a healthcare provider's guidance.
  • Approved MS subtypes are broadly consistent between the US and EU labels.
  • Plegridy is usually self-injected at home using a prefilled pen or syringe after training from a healthcare provider, though the first doses are often given or supervised in clinic.
  • It is not used for other autoimmune or infectious conditions.

Side Effects

  • The most common Plegridy side effects, reported in more than 1 in 10 people, are injection-site redness, flu-like symptoms, fever, headache, muscle aches, chills, tiredness, itching at the injection site, and joint pain; these are usually most noticeable after starting treatment and around dose increases.
  • Plegridy can cause serious liver injury, including hepatitis and rare severe liver failure, so liver function tests are monitored during treatment.
  • It can also cause or worsen depression, including suicidal thoughts — report any low mood or thoughts of self-harm to your healthcare provider immediately.
  • Serious allergic reactions, including anaphylaxis, have occurred in under 1% of patients; stop the injection and seek urgent help for swelling of the face or throat, difficulty breathing, or hives.
  • Interferon beta medicines can also trigger seizures in susceptible patients.
  • There is no boxed warning on the current Plegridy label.
  • Rotate injection sites to reduce skin reactions.

Precautions & Warnings

  • Before starting Plegridy, liver function should be checked, and it should be used cautiously, with closer monitoring, in patients with pre-existing liver disease, depression, or a seizure disorder, since Plegridy can worsen all three.
  • Regular blood counts and liver function tests are recommended during treatment.
  • Plegridy must be given by subcutaneous (or, in some markets, intramuscular) injection only, never intravenously, and injection sites should be rotated between the abdomen, thigh, and back of the upper arm to reduce skin reactions and, rarely, tissue damage (injection-site necrosis).
  • Report any signs of depression or suicidal thoughts to a healthcare provider right away; treatment may need to be stopped.
  • Live vaccines should be discussed with a healthcare provider before use during Plegridy treatment.
  • There is no absolute contraindication for pregnancy, but use should be discussed with a specialist, as should breastfeeding and fertility plans.
  • Driving is not specifically restricted, but flu-like symptoms may affect some patients.

Avoid Interactions

  • Formal drug-interaction studies with Plegridy are limited, but interferon beta medicines like Plegridy can reduce the activity of liver enzymes that metabolize certain other drugs, so caution is advised when combining it with medicines that have a narrow safety margin and are broken down by the liver, such as some anti-epileptic drugs; monitoring may be needed.
  • Because Plegridy can affect liver function and blood counts, combining it with other medicines that carry liver or bone-marrow risks should be discussed with a healthcare provider, who can advise on additional monitoring.
  • Live vaccines are generally best avoided or discussed with a healthcare provider before use during treatment, as response to vaccination may be altered.
  • As with all injectable medicines, do not mix Plegridy with other products in the same syringe.
  • Tell your healthcare provider and pharmacist about all prescription and over-the-counter medicines, herbal products, and supplements you take before starting Plegridy.
