Indications & Clinical Uses

  • Tapazole 10 mg tablets contain methimazole, an antithyroid medicine manufactured by Pfizer.
  • Tapazole is approved for the treatment of hyperthyroidism caused by Graves' disease or toxic multinodular goitre in patients for whom surgery or radioactive iodine therapy is not appropriate, and to control hyperthyroid symptoms and normalise thyroid function before thyroidectomy or radioactive iodine treatment.
  • Tapazole works by blocking the thyroid gland's production of new thyroid hormone; it does not affect hormone already stored in the gland, so improvement in symptoms can take several weeks.
  • Tapazole is distinct from propylthiouracil (PTU), another antithyroid medicine that carries its own boxed warning for liver failure; the two drugs are not interchangeable, and your healthcare provider will choose based on your trimester of pregnancy, if applicable, and other individual factors.
  • Tapazole is taken by mouth, usually once or twice daily, under the supervision of a healthcare provider who monitors your thyroid function.

Side Effects

  • Common side effects of Tapazole include skin rash, hives, nausea, vomiting, stomach upset, joint pain, numbness or tingling, loss of taste, hair loss, muscle aches, headache, drowsiness and dizziness.
  • Tapazole's current US label does not carry a boxed warning, but it does carry serious warnings you must know: agranulocytosis, a dangerous drop in infection-fighting white blood cells, can develop suddenly, so report fever, sore throat, mouth ulcers or any signs of infection to your healthcare provider immediately and stop the medicine until you are assessed.
  • Liver problems, including hepatitis, and rare cases of vasculitis have also been reported; seek prompt medical attention for yellowing of the skin or eyes, dark urine, unexplained bruising, or new joint pain with rash.
  • Blood counts and liver function are checked periodically during treatment.

Precautions & Warnings

  • Tapazole should not be used if you are allergic to methimazole or any of its ingredients.
  • Before starting, tell your healthcare provider if you are pregnant or planning pregnancy: methimazole used in the first trimester has been linked to rare birth defects such as aplasia cutis and craniofacial malformations, so your provider may prefer a different antithyroid medicine early in pregnancy and switch to Tapazole later if needed; use the lowest effective dose if it must be used.
  • Tapazole passes into breast milk, so discuss breastfeeding with your provider.
  • Report fever, sore throat or mouth sores at once, as these can be early signs of agranulocytosis, and have your white blood cell count checked if they occur.
  • Tapazole is an oral tablet only; it is not given by injection.
  • Liver function and thyroid hormone levels should be monitored throughout treatment.

Avoid Interactions

  • Tapazole increases the effect of oral anticoagulants such as warfarin as thyroid hormone levels fall, so your prothrombin time or INR should be monitored and the anticoagulant dose adjusted as needed.
  • As you become euthyroid on Tapazole, the doses of beta-blockers, digoxin and theophylline used to manage hyperthyroid symptoms often need to be reduced, since their clearance and effect change along with your thyroid status.
  • There is no specific requirement to avoid particular foods, alcohol or grapefruit with Tapazole.
  • Tell your healthcare provider and pharmacist about all prescription and over-the-counter medicines and supplements you take, especially blood thinners, heart-rate medicines and other thyroid treatments, so doses can be adjusted safely as your thyroid function changes during treatment.
