Indications & Clinical Uses
- Tekturna HCT 150mg/12.5mg tablets combine aliskiren, a direct renin inhibitor, with hydrochlorothiazide, a thiazide diuretic, currently labeled by Noden Pharma USA, Inc.
- Tekturna HCT is indicated for the treatment of hypertension, either in patients not adequately controlled on aliskiren or hydrochlorothiazide alone, or as initial therapy in those likely to need more than one drug to reach their blood pressure goal.
- It is a fixed-dose combination and a different formulation from Tekturna alone (Sr 3339, aliskiren only) - the two must not be confused, since Tekturna HCT adds a diuretic effect and its own set of precautions that Tekturna alone does not carry.
Side Effects
- Boxed warning: drugs acting directly on the renin-angiotensin system, including the aliskiren in Tekturna HCT, can injure or kill a developing fetus, especially in the second and third trimesters - it must be stopped as soon as pregnancy is detected.
- Common effects include dizziness, diarrhea, flu-like symptoms, cough, vertigo, tiredness and joint pain.
- Seek urgent medical attention for swelling of the face, lips, tongue or throat with breathing difficulty, or for signs of very low blood pressure such as fainting, which is more likely if you become dehydrated.
Precautions & Warnings
- Tekturna HCT is contraindicated in patients with diabetes also taking an ACE inhibitor or ARB, in anuria (inability to pass urine), and in people with a sulfonamide allergy because of the hydrochlorothiazide component.
- Combined use with an ACE inhibitor or ARB is generally avoided, particularly when kidney function (creatinine clearance) is reduced.
- Kidney function, blood pressure and potassium levels should be monitored; the thiazide component can also affect blood sugar and uric acid levels.
- It is not recommended during pregnancy or breastfeeding, and must be discontinued as soon as pregnancy is confirmed.
Avoid Interactions
- Avoid cyclosporine and itraconazole, which raise aliskiren levels, and use NSAIDs cautiously, since they can increase the risk of kidney impairment when combined with Tekturna HCT.
- Combining it with an ACE inhibitor or ARB raises the risk of hypotension, high potassium and kidney dysfunction.
- Lithium levels can rise to toxic levels when taken with thiazide diuretics like the hydrochlorothiazide in this product, and cholestyramine or colestipol can reduce hydrochlorothiazide absorption if taken at the same time.
- Doses of diabetes medicines may need adjustment.
- Tell your healthcare provider about every medicine you take.
Frequently Asked Questions
Tekturna HCT combines aliskiren with the diuretic hydrochlorothiazide to treat high blood pressure, either when one drug alone isn't enough or as a starting combination therapy. It is not the same as Tekturna alone, which contains only aliskiren.
Take Tekturna HCT once daily, with or without food, at about the same time each day. Swallow the tablet whole with water, and avoid a high-fat meal close to your dose, since fatty food reduces how much aliskiren your body absorbs.
Common effects include dizziness, diarrhea, flu-like symptoms, cough and tiredness. Tekturna HCT carries a boxed warning for fetal toxicity and must be stopped as soon as pregnancy is detected; seek urgent help for facial or throat swelling.
Avoid cyclosporine and itraconazole, which raise aliskiren levels, and be cautious with NSAIDs and lithium, which interact with this combination. Do not combine Tekturna HCT with an ACE inhibitor or ARB, particularly if you have diabetes or reduced kidney function.
Take a missed Tekturna HCT dose as soon as you remember that day; if you don't remember until the next day, skip it and continue your regular schedule. Never take two doses at once to make up for a missed one.
No. Tekturna HCT adds the diuretic hydrochlorothiazide to aliskiren, while plain Tekturna contains aliskiren alone. Tekturna HCT carries extra precautions, including sulfonamide-allergy and anuria contraindications, that Tekturna alone does not require, so the two products are not interchangeable.
