Indications & Clinical Uses

  • Timolol GFS 0.5% (timolol maleate ophthalmic gel-forming solution) is the AB-rated generic equivalent of Timoptic-XE, made by Alcon Laboratories for Sandoz Inc.; it is a topical non-selective beta-blocker used to lower elevated pressure inside the eye in adults with ocular hypertension or open-angle glaucoma.
  • Unlike a plain aqueous eye drop, the solution thickens into a gel on contact with the tear film, giving sustained drug release from a single drop instilled once daily, so it must be shaken before each use.
  • This is a genuinely different formulation from the twice-daily aqueous Timoptic solution and should not be confused with it or dosed the same way.
  • Timolol GFS is available in 0.25% and 0.5% strengths and is taken at home by the patient, not administered in a clinic.

Side Effects

  • Transient blurred vision lasting 30 seconds to 5 minutes after instillation affects roughly one in three patients, and burning or stinging affects about one in eight; other reported effects include eye irritation, dryness, and headache.
  • Timolol GFS has no boxed warning, but as a beta-blocker it can be absorbed into the bloodstream and cause serious effects in susceptible people: slow or irregular heartbeat, worsening breathing difficulty or wheezing in those with asthma or COPD, and worsening of heart failure.
  • Seek urgent care for chest pain, severe shortness of breath, or a very slow pulse.
  • People with diabetes should know it can mask the early warning signs of low blood sugar.

Precautions & Warnings

  • Timolol GFS must not be used by anyone with bronchial asthma or a history of it, severe chronic obstructive pulmonary disease, a slow heart rate (sinus bradycardia), second- or third-degree heart block, overt heart failure, cardiogenic shock, or hypersensitivity to its ingredients.
  • Use with caution in diabetes, since it can hide signs of hypoglycaemia, and in hyperthyroidism, since it can mask a fast heartbeat.
  • Remove contact lenses before instilling a dose and wait at least 15 minutes before reinserting them.
  • Pressing gently on the inner corner of the eye after each drop reduces the amount absorbed into the body.
  • Shake the bottle well before each use, since the gel-forming ingredient can settle.

Avoid Interactions

  • Tell your healthcare provider and pharmacist about all medicines and supplements you use.
  • Taking Timolol GFS with an oral beta-blocker can add to its effects on heart rate and blood pressure, and combining it with certain calcium channel blockers raises the risk of heart conduction problems.
  • Catecholamine-depleting drugs such as reserpine can cause an additive drop in blood pressure and slow heart rate, and CYP2D6 inhibitors can increase how much timolol reaches the bloodstream.
  • Do not use Timolol GFS together with a second topical beta-blocker eye drop, since this adds no benefit and increases side effects.
