Indications & Clinical Uses

  • Tradjenta 5 mg tablets contain linagliptin, a DPP-4 inhibitor.
  • In the United States, Tradjenta is approved, together with diet and exercise, to improve blood sugar (glycaemic) control in adults with type 2 diabetes; it can be used alone or added to metformin, sulfonylureas, pioglitazone or insulin.
  • It is not for type 1 diabetes or diabetic ketoacidosis.
  • The same molecule is marketed as Trajenta in the European Union, the UK and most other markets outside the US, where Boehringer Ingelheim is the sole marketing-authorisation holder; in the US, Tradjenta is co-marketed under the Boehringer Ingelheim-Eli Lilly diabetes alliance.
  • Linagliptin is also sold combined with metformin (Jentadueto) or empagliflozin (Glyxambi).

Side Effects

  • Tradjenta has NO boxed warning.
  • Common effects include a stuffy or runny nose, sore throat and cough (nasopharyngitis), diarrhoea and headache; low blood sugar (hypoglycaemia) is more likely when Tradjenta is combined with a sulfonylurea or insulin.
  • Serious but less common reactions to know include acute pancreatitis - persistent severe stomach pain, sometimes spreading to the back, with or without vomiting, needs urgent medical review; severe joint pain that can be disabling; heart failure symptoms such as increasing breathlessness, swelling of the ankles or rapid weight gain; and bullous pemphigoid, a blistering skin condition that has been specifically linked to DPP-4 inhibitors like linagliptin.
  • Seek urgent care for facial or throat swelling, difficulty breathing, or widespread skin blistering.

Precautions & Warnings

  • Tradjenta is not for treating type 1 diabetes or diabetic ketoacidosis.
  • It should be stopped if pancreatitis is suspected, and used cautiously in people with a history of pancreatitis, gallstones or high triglycerides.
  • Unlike some other DPP-4 inhibitors, linagliptin is cleared mainly through the bile and gut rather than the kidneys, so no dose adjustment is generally needed for kidney impairment, though this should still be confirmed with your prescriber.
  • Report any new heart-failure symptoms, since DPP-4 inhibitors as a class carry this caution.
  • Discuss any personal or family history of blistering skin conditions before starting.
  • Data in pregnancy and breastfeeding are limited, so alternative, better-studied treatments such as insulin are generally preferred during pregnancy.

Avoid Interactions

  • Strong inducers of the P-glycoprotein and CYP3A4 pathways, most notably rifampicin, significantly lower linagliptin levels and can reduce its blood-sugar-lowering effect; this combination should generally be avoided or blood sugar monitored more closely.
  • Combining Tradjenta with a sulfonylurea (such as glimepiride) or with insulin increases the risk of hypoglycaemia, and a lower dose of the sulfonylurea or insulin may be needed.
  • No dose adjustment is required with metformin.
  • There is no specific interaction with alcohol, but excessive alcohol increases the risk of low blood sugar and pancreatitis independently.
  • Tell your healthcare provider and pharmacist about every prescription, over-the-counter and herbal medicine you take, especially other diabetes treatments, so your dose can be adjusted safely.

Frequently Asked Questions

Tradjenta (linagliptin) treats type 2 diabetes in adults, alongside diet and exercise, by helping the body release more of its own insulin after meals. It can be used alone or added to other diabetes medicines such as metformin or insulin. It is not for type 1 diabetes.
