Indications & Clinical Uses
- Travatan Z (travoprost ophthalmic solution) 0.004% is Alcon's benzalkonium chloride (BAK)-free reformulation of travoprost, FDA-approved to reduce elevated intraocular pressure in adults with open-angle glaucoma or ocular hypertension; it was approved under its own NDA 21-994 with a BAK-free sofZia-preserved formula, distinct from the original, now-discontinued Travatan brand described separately in this catalogue.
- It is manufactured and marketed by Alcon Laboratories, Inc., part of Alcon Inc., which has been fully independent of Novartis since the two companies' April 2019 spin-off; the sheet's listing of 'Novartis' as manufacturer reflects an outdated ownership record and is corrected below.
- Travatan Z is given as one drop in the affected eye(s) once daily in the evening.
- It should not be substituted interchangeably with the original BAK-preserved Travatan or with other prostaglandin analogues without your eye specialist's guidance, since the preservative systems differ.
Side Effects
- Like the original formulation, Travatan Z's most common side effect is eye redness (ocular hyperaemia), reported in about 30% to 50% of patients; decreased visual acuity, eye discomfort and pain occur in roughly 5% to 10%.
- There is no boxed warning on the label.
- Travoprost can cause a gradual, likely permanent darkening of the iris and longer, thicker or more numerous eyelashes, which are usually reversible after stopping.
- Periorbital skin darkening can also occur.
- Less common but more serious effects include intraocular inflammation (uveitis), macular oedema in patients without a natural or intact lens, and bacterial keratitis from a contaminated bottle tip.
- Because Travatan Z is BAK-free, it is generally better tolerated by people who develop irritation from BAK-preserved eye drops, but redness and eyelash or iris pigmentation changes are not prevented by the different preservative.
- Seek prompt eye-care review for new pain, worsening redness, vision changes or discharge.
Precautions & Warnings
- Travatan Z has no absolute contraindications listed on its label, but it should be used cautiously in patients with intraocular inflammation, in aphakic or pseudophakic patients or those with other risk factors for macular oedema, and in anyone with a history of herpetic keratitis.
- Because Travatan Z uses the sofZia preservative system (boric acid, propylene glycol, sorbitol and zinc chloride) rather than benzalkonium chloride, it is generally suitable for contact lens wearers with less lens-related concern than the BAK-preserved original, though it remains good practice to remove lenses before instilling a drop and reinsert them at least 15 minutes later.
- Avoid letting the bottle tip touch the eye, eyelid or any surface to reduce the risk of bacterial keratitis from contamination.
- Discuss use in pregnancy and breastfeeding with your eye specialist, as data are limited.
- Tell your optometrist or ophthalmologist you use Travatan Z before any eye exam, since it can permanently change iris colour and eyelash appearance.
Avoid Interactions
- Tell your eye specialist about every eye drop, medicine and supplement you use before starting Travatan Z (travoprost).
- If you need more than one type of eye drop, space different products at least 5 minutes apart so each is properly absorbed.
- There are no known clinically significant systemic drug interactions with travoprost at the low doses absorbed from an eye drop, but avoid using other prostaglandin-analogue drops, such as latanoprost, bimatoprost or tafluprost, at the same time as Travatan Z, since combining two drugs from the same class does not improve pressure control and can add side effects.
- Do not switch between Travatan Z and the original BAK-preserved Travatan, or between brands of generic travoprost, without checking with your eye specialist, since the preservative systems differ.
- Do not let the dropper tip touch your eye or any surface, and never share your eye drops.

Frequently Asked Questions
Travatan Z (travoprost) 0.004% eye drops lower high eye pressure in adults with open-angle glaucoma or ocular hypertension. It's Alcon's benzalkonium chloride (BAK)-free version of travoprost, made for people who prefer a preservative-free-feeling option compared with the original, now-discontinued Travatan.
Use one drop of Travatan Z in the affected eye, usually once daily in the evening. Wash your hands first, avoid touching the dropper tip to your eye, and wait at least 5 minutes before using any other eye drop.
Eye redness affects 30% to 50% of people using Travatan Z (travoprost); mild eye discomfort and blurred vision are also common. It can permanently darken the iris and lengthen eyelashes over time. Get urgent eye care for new pain, worsening redness or sudden vision changes.
Avoid combining Travatan Z (travoprost) with other prostaglandin-analogue eye drops, such as latanoprost or bimatoprost, since this adds side effects without improving pressure control. Don't switch to the original BAK-preserved Travatan or a different generic without checking with your eye specialist first.
Alcon makes and markets Travatan Z. Alcon has been fully independent of Novartis since their April 2019 spin-off, so any listing showing Novartis as the manufacturer is outdated; Alcon Laboratories, Inc. is the current company behind this product.
Travatan Z contains travoprost 0.004% (0.04 mg/mL), the same strength as the original Travatan. Store it between 2°C and 25°C, and discard the bottle 6 weeks after first opening it, even if some solution remains, to avoid using contaminated drops.
