Indications & Clinical Uses
- Trintellix 20mg (vortioxetine), originally developed by Lundbeck of Denmark and co-marketed with Takeda, is an oral antidepressant approved by the FDA for the treatment of major depressive disorder (MDD) in adults.
- It belongs to a class sometimes called a serotonin modulator, acting on several serotonin receptors as well as inhibiting serotonin reuptake, which distinguishes its mechanism from standard SSRIs.
- It is taken once daily, with or without food, usually starting at 10mg and adjusted by a healthcare provider up to a maximum of 20mg daily; full antidepressant benefit can take several weeks to appear.
- Trintellix was renamed from its original US brand name 'Brintellix' in 2016 to avoid confusion with the unrelated heart medicine Brilinta; the molecule and formulation are unchanged.
- It is approved for adults only; safety and effectiveness in children have not been established.
Side Effects
- The most common Trintellix side effect is nausea, affecting roughly a fifth to a third of patients depending on dose, especially in the first weeks; other frequent effects include constipation, vomiting, dizziness, dry mouth and unusual dreams, and sexual side effects such as delayed orgasm can occur.
- Trintellix carries a boxed warning shared by all antidepressants: it can increase suicidal thoughts and behaviour in children, adolescents and young adults up to age 24, particularly in the first months of treatment or after a dose change, so close monitoring for worsening mood or new suicidal thinking is essential.
- Rare but serious reactions include serotonin syndrome (agitation, fast heartbeat, sweating, muscle twitching), abnormal bleeding, activation of mania, and a severe drop in blood sodium.
- Seek urgent medical help for any of these serious symptoms.
Precautions & Warnings
- Trintellix carries a boxed warning for increased suicidal thoughts and behaviour in patients under 25, so all patients need close monitoring, especially early in treatment.
- It must not be combined with a monoamine oxidase inhibitor (MAOI) or started within 21 days of stopping one, because of the risk of serotonin syndrome.
- Use with caution in people with a bleeding disorder or those taking blood thinners, since Trintellix can increase bleeding risk, and in people with a history of mania, narrow-angle glaucoma or a seizure disorder.
- It can lower blood sodium (hyponatraemia), especially in older adults, and stopping it suddenly after regular use can cause discontinuation symptoms, so any dose change should be gradual and supervised.
- Discuss the risks and benefits of continuing Trintellix during pregnancy or breastfeeding with your healthcare provider, since untreated depression itself carries risk.
Avoid Interactions
- Never combine Trintellix with an MAOI, or start one within 21 days of stopping Trintellix, because of the serious risk of serotonin syndrome.
- Other serotonergic medicines - including SSRIs, SNRIs, triptans, tramadol, St. John's Wort and lithium - raise this same risk and should only be combined under medical supervision.
- NSAIDs, aspirin, warfarin and other blood thinners increase the risk of bleeding when taken with Trintellix.
- Strong inhibitors of the liver enzyme CYP2D6, such as bupropion, fluoxetine, paroxetine and quinidine, can raise Trintellix levels, and the dose may need to be halved; strong CYP inducers, such as rifampicin, can lower its levels and may need a higher dose.
- Alcohol can add to dizziness and drowsiness.
- Tell your healthcare provider about every prescription, over-the-counter medicine, herbal product and supplement you take.

Frequently Asked Questions
Trintellix (vortioxetine) treats major depressive disorder in adults. It works on several serotonin pathways in the brain to improve low mood, and is taken once daily. It can take several weeks of use before you notice the full antidepressant effect.
Take Trintellix once daily, with or without food, at a dose your provider sets between 10mg and 20mg. Swallow the tablet whole. Do not stop suddenly once you're on a regular dose - your provider will taper it gradually if treatment ends.
Nausea is the most common Trintellix side effect, along with constipation, dizziness and dry mouth. Trintellix carries a boxed warning: it can increase suicidal thoughts in people under 25, so report any worsening mood or new suicidal thoughts to your provider right away.
Never take Trintellix with an MAOI or within 21 days of stopping one - the combination can cause dangerous serotonin syndrome. Avoid other serotonergic drugs, blood thinners and heavy alcohol use without medical advice, since they raise bleeding or serotonin syndrome risk.
The US brand name changed from Brintellix to Trintellix in 2016 after mix-ups with the unrelated heart drug Brilinta caused dosing errors. Vortioxetine, the active ingredient, and the tablet itself did not change - only the brand name did.
No. Trintellix is approved for major depressive disorder in adults only; safety and effectiveness in children and adolescents have not been established. Antidepressants, including Trintellix, carry a boxed warning about increased suicidal thoughts in people under 25.
