Indications & Clinical Uses
- Trulicity 1.5 mg/0.5 mL is a single-dose pre-filled pen containing dulaglutide, a glucagon-like peptide-1 (GLP-1) receptor agonist given once weekly by subcutaneous injection.
- In the United States it is approved, alongside diet and exercise, to improve blood sugar control in adults and children 10 years and older with type 2 diabetes, and separately to reduce the risk of major cardiovascular events (heart attack, stroke or cardiovascular death) in adults with type 2 diabetes who have established cardiovascular disease or multiple cardiovascular risk factors.
- It is not for use in type 1 diabetes or diabetic ketoacidosis.
- Trulicity is manufactured by Eli Lilly and Company and is available in 0.75 mg, 1.5 mg, 3 mg and 4.5 mg weekly pen strengths.
Side Effects
- Trulicity carries a boxed warning: in rodent studies dulaglutide caused thyroid C-cell tumors, and it is not known whether this occurs in humans, so Trulicity is contraindicated in anyone with a personal or family history of medullary thyroid carcinoma or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2).
- The most common side effects, affecting more than 5% of people, are nausea, diarrhea, vomiting, abdominal pain and decreased appetite, which are usually worst when starting or increasing the dose.
- Report a lump or swelling in the neck, hoarseness, trouble swallowing, or shortness of breath, which could signal a thyroid tumor, and seek urgent care for severe stomach pain that may spread to the back (possible pancreatitis) or signs of a serious allergic reaction.
Precautions & Warnings
- Do not use Trulicity if you or a family member has had medullary thyroid carcinoma or MEN 2, in line with its boxed warning, and stop treatment if a thyroid tumor is suspected.
- Trulicity is not recommended in people with severe gastrointestinal disease such as gastroparesis, and it should be used with caution if you have a history of pancreatitis or diabetic retinopathy, which should be monitored during rapid glucose improvement.
- It is for subcutaneous injection only, into the abdomen, thigh or upper arm, and must never be given intravenously or intramuscularly.
- Discuss use in pregnancy and breastfeeding with your healthcare provider, as data are limited.
- Store new pens refrigerated at 2°C to 8°C; a pen may be kept at room temperature, below 30°C, for up to 14 days if refrigeration is not available, then must be discarded.
Avoid Interactions
- Tell your healthcare provider about all medicines you take, including insulin and other diabetes treatments, before starting Trulicity.
- Trulicity slows stomach emptying and can reduce or delay the absorption of oral medicines taken at the same time, so tell your prescriber about any oral drugs with a narrow safety margin.
- Using Trulicity together with insulin or with sulfonylureas (insulin secretagogues) increases the risk of low blood sugar (hypoglycemia), and a dose reduction of the other medicine may be needed; check your blood sugar as advised.
- There is no need to avoid specific foods, but excess alcohol can increase the risk of low blood sugar and pancreatitis.
- Always tell a new prescriber or pharmacist that you use Trulicity.

Frequently Asked Questions
Trulicity (dulaglutide) is a once-weekly injection that helps control blood sugar in adults and children 10 and older with type 2 diabetes. It is also used to lower the risk of heart attack, stroke and cardiovascular death in adults with type 2 diabetes and existing heart disease or risk factors.
Trulicity is injected under the skin once a week, any time of day, with or without food, using a single-dose pre-filled pen. Inject into your abdomen, thigh or upper arm, rotating sites. It comes in 0.75 mg, 1.5 mg, 3 mg and 4.5 mg strengths.
The most common Trulicity side effects are nausea, diarrhea, vomiting, stomach pain and reduced appetite, especially early in treatment. Trulicity has a boxed warning for thyroid C-cell tumors seen in animal studies; report any neck lump, hoarseness or trouble swallowing right away.
Never use Trulicity if you or a family member has had medullary thyroid cancer or MEN 2 syndrome, per its boxed warning. Combining it with insulin or sulfonylureas raises the risk of low blood sugar, and it can slow absorption of other oral medicines, so tell your doctor about everything you take.
If you miss a Trulicity dose and at least 3 days remain until your next one, inject it as soon as you remember; otherwise skip it and resume your regular weekly schedule. Never take two doses within 3 days of each other to make up for a missed dose.
Yes, Trulicity is approved for type 2 diabetes in children aged 10 and older, at the same weekly doses used in adults. It is not approved for type 1 diabetes. Discuss pregnancy plans with your healthcare provider, as dulaglutide is generally not recommended during pregnancy.
